On 5/4/26, appropriate care and supervision was not provided to Child A when program staff left Child A inside the classroom asleep while they went outside with the classroom. The program staff did not realize Child A was not with them until after they were lining up to come back inside from their outdoor time. It is estimated after interviewing program staff that Child A was unsupervised from 20-30 minutes.

The center did not provide appropriate supervision when Teacher 1, Teacher 2, and Teacher 3 were on the other side of the gym while Child A was waiting in line for the bathroom. Child A's finger was shut in the bathroom door when a child open the door as a result a part of the tip of the finger was not connected and needed to be sewn back on to the finger.
