On 5/12/2022 licensee designee Katelyn Barton reported that on 5/8/2022 they received an email from Child A's Mother. Child A's Mother stated that Child A stated that while at the child care, she and Child B were laying under a blanket by the bean bag chairs in the lower level preschool classroom. She stated that Child B asked her to play a game called "touching P's and butts." Child B then pulled down her pants and began touching her own privates, and asked Child A to pull her pants down and do the same. Child A's Mother stated that after an undetermined amount of time, Child A stated that she was bored with the game, pulled up her pants, and went to play something else. Child A's Mother expressed that she is concerned about why Child B is acting out in this manner and does not want Child B to have any unsupervised contact with Child A in the future. Ms. Barton stated that she has spoken to the child care staff members in the classroom, and they denied that the children are allowed to play under blankets at any time. They denied knowledge of any incident of Child A and Child B laying under a blanket together. They removed any possible blankets, and the bean bag chairs from the classroom. They are increasing their supervision to assure no future incidents. Ms. Barton reported that Child A's Mother was happy with their response and both children remain enrolled and in attendance. Ms. Barton also reported that as of 5/20/2022 she will no longer be the licensee designee for the child care facility. On 5/13/2022 I completed an unannounced onsite inspection and interviews with child care staff members Ronniece Erwin and Cheryl Curry. Ms. Erwin stated that she is the lead caregiver in the classroom and is aware of the reported allegations. She expressed that she never observed Child A and Child B laying under a blanket together. She stated that they only get the blankets out at nap time for the children to use on their cots. Ms. Erwin indicated that many of the child care children like to go to the quiet, book area and lay on bean bag chairs together. Since this allegation, she has removed 3 the bean bag chairs and rearranged the classroom to make things even more visibly open. Ms. Erwin reported that she has never observed Child A or Child B act out in a sexual manner. She reported that neither Child A or Child B have behavioral issues. She denied concerns with either child, and denied any child reported this incident to her or other child care staff. Ms. Erwin reported that Child B is quite affectionate and will pat or hug you, but nothing sexual or inappropriate. Ms. Erwin has spoken to Child A and Child B's Mothers about this concern. Child A's Mother was mostly concerned about why Child B was sexually acting out. She expressed that Child A's Mother was happy with the changes they made in the classroom to address these concerns. Child B's Mother has denied any issues or history of Child B acting out in a sexual manner. Ms. Curry is a child care staff member in the lower level preschool classroom. She works each day from 8-3pm. She is aware of the reported allegations. Ms. Curry denies any knowledge of Child A and Child B laying under a blanket together. She stated that they do not allow children to lay under blankets together. Ms. Curry expressed that may have had some spare blankets accessible in the classroom, but those have now been removed. She stated that Child A and Child B did periodically lay down together on the bean bag chairs to read books or play together. The bean bag chair was really worn down and the children would sink down into the chair. They have removed the chair since the allegation was made. Ms. Curry denied ever observing Child A or Child B act out in a sexual manner. She denied any child reported this incident or concern to her. She reported that Child B is a "touchy" and affectionate child, but nothing sexual in nature or inappropriate. On 6/16/2022 I completed phone contact with Child A's Mother. Child

On 12/2/2021 I completed onsite interviews with licensee designee Katlyn Barton and child care staff members Cheryl Curry and Ronniece Erwin. Ms. Barton reported that on 12/1/2021 she was called by another child care staff member, Callie Hull who also has a child enrolled in the center, and was informed that when she arrived at the classroom to pick her child up, Child A was locked in the room alone and scared. The door locks from the outside and requires a key to enter but will open from the inside when locked. Ms. Barton called Ms. Erwin immediately and they discovered that when the classroom left for the upstairs gym, they had accidentally left Child A in the classroom and did not realize Child A was not with them in the gym. They had been in the gym for approximately 25 minutes by the time the error was discovered. Ms. Barton reported that Ms. Curry and Ms. Ewin were both given disciplinary action, and they have protocol to follow to ensure this does not happen that was not followed on 12/1/21. Ms. Barton said the child care staff members are to use the classroom ipad to "roll call" each child when they line them up at the door, and check off their name as they verify the child is in line. On 12/1/21 the ipad was charging and not used. The staff also are supposed to count the children when they arrive to the gym, and they did not do this either. Ms. Curry was visibly upset during my interview with her. She said that typically Ms. Erwin has the ipad and calls each child's name while they line up at the door. However, on 12/1/21 things in the classroom were off rhythm all throughout the morning because there was a child that was having a hard day, and they were giving this child extra attention. Ms. Curry said she is not trying to make excuses; just explaining how they believe the situation created itself. Ms. Curry had the children using the bathroom just before lining up. She asked Child A to go to the bathroom at least three times, each time Child A would say in just a minute. Meanwhile, Ms. Curry continued to excuse other children to the bathroom which is located in the classroom. Ms. Curry believes that Child A must have been in the bathroom when they exited the classroom and no one realized it, because Child A was not visible in the classroom when they left. Ms. Curry said now they not only count heads with the ipad before leaving but they check the bathrooms, and count on their way to the gym and when they arrive at the gym. Ms. Erwin was also visibly upset during my interview with her. She reported that she can't believe this happened to her because she has been a child care provider for 15 years and nothing like this has ever happened. Ms. Erwin admitted that she was not her best on 12/1/21 due to both personal reasons and difficulty with a child in the classroom that was also having a bad day. Ms. Erwin said that while they were lining up the child that was struggling throughout the day was being really difficult and she was focused on making sure the child did not harm another child. She believes this is why she forgot to grab the ipad because of the distraction. Ms. Erwin said that Ms. Curry was helping children use the bathroom prior to lining up, and she thinks this is 3 where Child A must have been when they left the classroom. She confirmed that they had been in the gym for 25 minutes before Ms. Barton called her and asked why Child A was left in the classroom. Ms. Erwin said she has no idea how either one of them missed that Child A was not with them. The classroom had 12 children at the time that they went to the gym. They did not do a head count when they arrived. On 12/3/21 I spoke to Child A's mother, Child B's mother and Child C's mother. Child A's mother reported that they will continue to keep child A enrolled and understand that accidents can happen. She said Child A's father was really upset because he (Child A's father) was left alone on accident once as a child and it was really traumatizing for h
