On 1/6/2021 I completed an unannounced onsite inspection and interview with program director Kimberly Wolfersberger. She reported that the child care facility has an electronic attendance system. All parents are required to use a fingerprint to enter the building at arrival and departure to check out their children from care. This system maintains the accurate arrival and departure times for all children. Ms. Wolfersberger indicated that the system also breaks down the attendance for each classroom. She denied any issues with the attendance record system failing to work. During the onsite inspection, I reviewed the classroom attendance for all classrooms. Ms. Wolfersberger also showed me the last several days of attendance. The electronic attendance record was accurate for all classrooms. Ms. Wolfersberger reported that each classroom also has an electronic daily report system for all children. This application allows them to communicate diaper changes, naps, meals, and learning activities. The program also allows the child care staff members to directly message parents for any additional needs. She reported that this particular application is not how they maintain attendance for each classroom or the building. On 2/11/2021 I completed phone contact with Child B's Mother. Child B has been enrolled at the child care for approximately a 1 years. Child B's Mother denied any '/2 issues with attendance at the child care facility. She denied ever noting any discrepancies in the attendance. She expressed that the child care facility has a system in place that requires the parents to check in their children at arrival, and check them out at departure. She reported this is all done electronically. On 2/11/2021 I completed phone contact with Child C's Father. Child C has been enrolled at the child care for approximately 2 years. Child C's Father denied any issues or concerns with the attendance keeping at the child care. He reported that parents are required to use a fingerprint to enter and exit the building and to check their child in and out of care. He has never observed any discrepancies or issues with the attendance system. I attempted phone contact with Child A's Mother, but no messages could be left and no return calls were received.

On 9/30/2020 I had phone contact with Child A's Mother. She reported that on earlier this date, the child care center sent her a note that Child A had drawn on his face and arms with a red Sharpie marker. She was told that child care staff wiped the marker off with his Kirkland brand baby wipes. She was also told that the marker came off, but within a few minutes raised red blotches started to develop. They put an ice pack on the areas, and recommended she come and get 3 him. They also sent her photos of the areas. Child A's Mother forwarded me a copy of the note she was sent, and multiple pictures of Child A's marks. I observed four pictures of Child A taken earlier this same date. Child A appears to be a very fair skinned child. Child A had a raised, red, blotchy mark on the left side of his face from his scalp to his lower cheek. He had the same marks on his forearm, and the top of his head. Child A's Mother stated that the family has always used Kirkland baby wipes on Child A, and he has never had a reaction before. She believes that the child care used a cleaning solution of some kind on his skin and that he now has chemical burns. She does not believe that the marks are from the baby wipes or the Sharpie. Child A's Mother stated that Child A should not have had access to the Sharpie marker as it is toxic and should be not be accessible to children. Further, she is upset that Child A had the time and ability to write all over is body before he was discovered. Child A's Mother took Child A to the emergency room for examination after she left the child care on the same date. She forwarded me a copy of the medical examination. The report indicated that Child A was having an allergic reaction, and medication was prescribed. Child A's Mother is following up with her pediatrician as soon as possible. Child A's Mother expressed that after she returned from the hospital, she and her husband used the baby wipes to scrub Child A's arm and no reaction occurred. She feels this indicates that another chemical or product was used and the child care is just not reporting. She stated that her mother is a md she is familiar with chemical burns. Her mother feels this is a chemical burn. I expressed that the medical report indicates that the marks are an allergic reaction, and not a chemical burn. On 9/31/2020 I reviewed the Sharpie website to determine if the markers were considered toxic or poisonous. The website states that the markers are non-toxic and are used to mark patients during medical procedures and surgeries. There is nothing on the website to indicate that the markers are not safe to be around children. On 10/1/2020 I completed phone contact with Child A's Mother. She also forwarded me updated photos of Child A's marks. The marks on his face appear more swollen and irritated. His eye is now also red and puffy. She is taking Child A to the pediatrician on this date. On 10/1/2020 I completed an unannounced onsite inspection at the child care center and interview with child care staff member Tabitha Hawkin. The program director Kimberly Wolfersberger was out of the building. Ms. Hawkin stated that child care 4 staff member Ashley Bowen is the lead caregiver in the classroom with Child A. She is currently on vacation. Ms. Hawkin reported that on 9/30/2020 the children in the classroom were laying down for nap. During this time, she walked over to Child A's cot area and observed him drawing on his body with a red Sharpie marker. She reported that Child A had drawn on his cot, face, head, and arms. She immediately took the Sharpie away from Child A and started wiping the areas with his Kirkland baby wipes. She stated that the wipes worked in removing the Sharpie marker. She denied scrubbing or rubbing Child A aggressively and indicated that she only wiped with normal pressure. Ms. Hawkin stated that she was surprised that the marker came off so easily. Prior to the onsite inspection, I observed the Sharpie website and it indicated that Sharpie markers are considered non-toxic and are used in medical procedures. During phone contacts with Child A's Mother, she expressed concern that Child A had access to the Sharpie marker. She believes that the marker is not safe for children. Child A's Mother did state that the marker does not seem to be the reason for Child A's reaction, as there are areas on his body where Sharpie marker was not cleaned off and there is no allergic reaction. During the onsite inspection I observed the Sharpie marker and it did not have a label stating that it was not safe to be around children. Ms. Hawkin and Ms. Wolfersberger expressed that they did not keep Sharpie marker's out of the reach of children, because they did not believe were poisonous or toxic in nature. I observed the medical notes from Bronson emergency room and and both indicated that the allergic reaction stemmed from either the Kirkland baby wipes or other potential product used to clean off Sharpie marker and not the marker alone.
