(10) Toilet learning or training must be planned cooperatively between the child's regular caregivers and the child's parent, so the toilet routine established is consistent between the center and the child's home.

    On 06/26/2023 I conducted an unannounced onsite inspection to Arbor Post Child Development Center (CDC). I observed eleven children present at the center with five staff. The children were eating snack in the infant and toddler rooms and having free choice time in the preschool room. I interviewed program director, Adriana (Ana) Black, Child Care Staff Member (CCSM) 1, and Child Care Staff Member (CCSM) 2. I obtained child information cards, the parent handbook, and children's attendance records. Ana Black reported that the main child care staff working in Child A's classroom are CCSM 2 and Child Care Staff Member (CCSM) 3. CCSM 1 also works in the classroom at times. Ana Black indicated she was unaware of this issue until last week when Child A and Child B's Mother brought it to her attention. She asked Child A and Child B's Mother why she just brought it to her attention, and said their mother said, "I understand things happen sometimes, so I just let it go." Ana Black has not noticed Child A smelling like feces and was not sure about him having sores in his buttocks as she has never observed him toileting. She asked child care staff who work with him, and they told her Child A does not typically have a bowel movement at the center, maybe twice a week. Ana Black denied that child care staff are not checking in on Child A when he uses the bathroom, and indicated every child is checked in on when they use the bathroom and given the tools/instructions/encouragement to finish wiping on their own if they are toilet trained or working on toilet training. She reported that the center encourages children to be independent as they are learning to use the toilet. If a child has an accident, they are cleaned up and changed immediately. CCSM 1 reported that Child A is toilet trained, so he goes to the bathroom on his own. She and other child care staff do check with him to see if he needs help when he is in there. All of the children in the classroom are preschool age, so they are working with them on learning to do tasks, like using the bathroom, independently, in preparation for starting school. CCSM 1 indicated she was unaware of his wiping being an issue until earlier today when Child A and Child B's Mother brought it to her attention and brought in extra underwear for Child A. CCSM 1 has not noticed Child A ever smelling like feces and has not observed any rash or sores on his buttocks, but also has not looked at his buttocks. Child A is a child that often comes right up in a person's space too, so she feels like a feces smell would have been noticed. CCSM 1 reported Child A has never complained about a rash or soreness. CCSM 2 confirmed that Child A is toilet trained and goes to the bathroom by himself as any child who is toilet trained is encouraged to do. Child care staff do check on him and the other children in the bathroom regularly and instruct them to make sure 3 and wipe or continue wiping if needed. If they do smell feces on a toilet trained child after they have used the bathroom, they will encourage them to go back into the bathroom and do some more wiping until the toilet paper is clean. CCSM 2 recalled only one time Child A smelling like feces and she took him to the bathroom to do some more wiping. CCSM 2 has not observed a rash or sores on/in his buttocks but indicated she does not examine him when he uses the bathroom as he is toilet trained. On 08/02/2023 I spoke with Child A, Child B, Child C, Child D, Child E, and Child F's Mothers. Child A and Child B's Mother does have concern with the center not helping Child A wipe after he uses the bathroom as she says he comes home smelling like feces at times, has feces crusted/caked in his buttocks, streaks of feces in his underwear, and rash/sores as a result. She sends him to the center with extra pairs of underwear and said he returns with the extra underwear provided on, so she is assuming he must still be having issues and changing underwear.     On 06/26/2023 I conducted an unannounced onsite inspection to Arbor Post Child Development Center (CDC). I spoke with program director, Ana Black, CCSM 1, and CCSM 2. Ana Black denied that children are not being served meals and snacks if they are present at the center. She reported that they follow a daily schedule with set meal and snack service times. All children are offered food and the center caters to children's allergies if they have a form on file with the CDC and Foundation for Behavioral Resources (FBR) commissary. Ana Black indicated the times that Child A arrives to the center vary and are sometimes after the breakfast is served and cleaned up. Many times, the class has moved onto a different activity, but if the food happens to be out, he is offered and served food if he wants to eat. CCSM 1 denied that children are not being offered/receiving a meal/snack if they are present during meal or snack time, and that she has not observed Child A not receiving a meal/snack. She reported that Child A often arrives later than breakfast time to the center, sometimes closer to lunch time, but that they still ask him if he wants a snack. Child A usually tells them no and waits to eat at snack or lunch with the other children. CCSM 1 indicated they have a daily schedule that includes the meal times, and any child that is at the center is offered and receives food. 5 CCSM 2 reported that breakfast is served at 8:00AM, lunch at 11:00AM, and snacks at 2:00PM and 5:00PM. She reported that Child A has always either been present for meals/snacks or came in and received a meal/snack that she has observed. CCSM 2 indicated that if they are made aware a child will be late a certain day, they will hold a meal/snack for that child, but it has to be arranged specifically with the center for that day. On this same day I reviewed the food policy in the parent handbook. Each parent receives and signs that they received the parent handbook. The food policy is the following, "The Child Care Food Service prepares all food for the center. We serve nutritious and balanced meals in which the use of salt, fats, sugars, coloring, and preservatives is minimized. The center provides: early riser: 6:00am-6:30am, breakfast: 8am-8:30am, lunch: 11:30-12:00pm, and snack: 2:30pm- 3:00pm. Monthly menus are posted in each classroom and are available to take home. Food substitutions will be noted on the day they occur and posted for parents to see in the lobby. Whole milk is served to children up to 2 years of age and 1 is served to 1/2 1/2% children over 2 years old. If a parent prefers to change this for their child, a written 1/2 request from the pediatrician is required." On 08/02/2023 I spoke with Child A, Child B, Child C, Child D, Child E, and Child F's Mothers. Child A and Child B's Mother has concerns with the center's food service. She reported that if her children are late, they are denied food, even if the food cart is still sitting out and other children are eating. Child C and D's Mother did not have concerns with the food service at the center. She reported that Child D attends a different school for part of the day, so she arranged with the center to hold a lunch for him to eat when he is dropped off by his school bus. Child E and Child F's Mother did not have concerns with the food service at the center. She reported that she is aware of the daily schedule for her children's classrooms and knows the meal times. If she wants her children to have a meal at the center, she makes sure they arrive in a timely manner. If she is late, she feeds her children ahead of time. On 08/07/2023 I spoke with CCSM 3. CCSM 3 denied that Child A is deprived a snack or meal if he is present at the center during meal/snack times. She indicated that there are times he may not receive breakfast as he does not arrive to the center until after breakfast was served and finished/cleaned up, and that his attendance at the center varies on times he arrives or is present.

    On 09/13/2022 I received an incident report from the Arbor Post Child Development Center (CDC) program director, Ana Black, and spoke with her via telephone. Ms. Black reported that the incident occurred on 09/12/2022 around 11:30AM in the infant room. She was not present at the center as she was out of town but the assistant program director, Toni Koons, was present. Child A was in a high-chair as it was lunch time and he was eating. Child A leaned forward and fell forward out of the high-chair, the tray going with him to the floor. Child A was not strapped/buckled in the chair. Child A did have a slight red mark on his head but the mark was gone after 20 minutes. Ms. Black indicated the child care staff present in the room were Brittany Malone, Sabrina Beebe, and Queashia Kindred. September 12, 2022, was Ms. Kindred's first day, so she was shadowing staff. Ms. Malone was the child care staff member that had put Child A in his high-chair to feed him lunch. At the time of the incident, there were seven children present and three child care staff. I asked Ms. Black if Child A was present today at the center, and she said yes, Child A is present and doing well today. Ms. Black indicated the center did write up Ms. Malone for not strapping and buckling Child A in his high-chair as it is policy at the CDC to buckle all children into any equipment or changing tables that have straps. On this same day I contacted Child A's Mother. Child A's Mother reported she received an incident report that she signed from the center that indicated Child A had fallen out of the high-chair due to a staff member not buckling him in the chair and making sure the tray was in place. According to the center, her child leaned forward in the high-chair and the tray became unlatched, Child A falling forward with the tray to the floor. Child A's Mother did not have concerns with the center, aside from this incident that occurred. She indicated that she does understand accidents happen but that strapping and buckling a child in a high-chair and checking to make sure the tray is latched in place is common sense and could have avoided this incident from completely happening. Child A's Mother confirmed that Child A is still attending the center. On 09/16/2022 I received an email from Ms. Black reporting that Ms. Malone resigned her position at the center and told Ms. Black she had taken employment somewhere else. Ms. Black gave me Ms. Malone's contact information. On 09/26/2022 I conducted an unannounced onsite inspection to Arbor Post CDC. I observed three children and three child care staff in the Explorers room. There was one child sleeping (Child A), one child in a low to the ground/small high-chair eating with a child care staff sitting next to them, and one child being held in a rocking chair. Child A did wake up from his nap while I was still in the room and was up, happily, playing with toys. There were both regular size high-chairs (with foxes on them), and low to the ground/small high-chairs. I obtained child information cards, attendance records, and the CDC parent handbook. I interviewed program director, Ms. Black, assistant program director, Toni Koons, and child care staff, Sabrina Beebe, Denise Lindstrom, and CaDaysha Roye. 4 Ms. Black confirmed that Child A is still attending the center and that he is present today if I want to see him in the classroom. She indicated the staff working today are Ms. Beebe, Ms. Lindstrom, and Ms. Roy. Ms. Black reported that one of the center rules is that if equipment or changing tables have straps/buckles, the child should be strapped/buckled in, and that this policy is covered in center orientation for new hires. Ms. Koons was present at the center when the incident occurred. She was in her office training a new employee when Ms. Beebe walked in and told her that Child A had fallen out of his high-chair because he was not buckled in and he leaned forward and fell with his tray.
