On 4/6/21, Child A's Mother said that when she dropped off Child A (31 month-old male) on 3/31/21 he was crying and she messaged staff member Krista Campbell afterwards to see if he was doing better. Child A is on the preschool side of the room and Krista Campbell is not his primary caregiver. She works on the infant and toddler side of the room but is Child A's godmother. Krista messaged her back that there was a lot of yelling going on there that day and that it was chaotic and she was going to quit. She informed Child A's Mother that the licensee, Allison Deiter, and the preschool teacher, Ann Chenier, were yelling at her and another staff member, Julie DeGrave, in front of the children. On 4/12/21, an unannounced visit was made to the center. Mrs. Deiter was interviewed about what took place 3/31/21. She had not put herself on the schedule to come in that day but made an unannounced visit to the center. She had concerns about infant toddler staff members Krista Campbell and Julie DeGrave and wanted to observe. She had been having problems with them being on their phones while caring for children. Mrs. Deiter said that staff members are not supposed to be texting parents or using their phones for personal reasons. Parents know to leave a message on the center phone and she will return their calls. On 3/30/21, Ms. DeGrave had been on her phone while on the playground with the children. Her other concern was that both of them liked to go to the ledge between the infant side and the preschool side and talk to the staff on the preschool side. She had been asking both of them not to do this. She felt both Ms. DeGrave and Krista Campbell had been disrespecting her lately and told them that. On 3/31/21, she saw Krista Campbell on her phone and she was showing her phone to Ms. DeGrave. She asked her Krista "Is there something on your phone I need to see?" Ms. DeGrave said "I haven't been on my phone". Krista said nothing. She reminded Ms. DeGrave that just the day before she had been talking on her phone right in front of her and that this is disrespectful. She reminded them that if there was something 3 important and they needed to use their phones, they could just ask her or tell her. Ms. DeGrave then asked "Why are you standing here watching us all the time?" Then preschool teacher Ann Chenier walked over to the three of them and said to Ms. DeGrave and Krista that she could not believe how they disrespected their boss and said mean things about her and that they were acting like bullies. She did raise her voice and told them in all her many years of teaching she had never seen anyone treat their boss like they do. Krista said "I don't care". There were children present on both the preschool and the infant side when these conversations took place. Ms. DeGrave said "if that's how you feel" and walked out. She had already given a two week notice and her last day was supposed to be in two more days. She still has not returned Mrs. Deiter's key. On 4/4/21, she received a message from Krista saying she was done. She had been on the schedule for 4/5/21. On 4/21/21, preschool teacher Ann Chenier was interviewed by phone. She was aware that Mrs. Deiter was having problems with the two staff members. On 3/31/21, she saw Krista and Ms. DeGrave were looking at Krista's phone and heard Mrs. Deiter confront them about this and heard them talk back to her in a sassy tone. She walked over to the three of them and admitted to yelling at them in front of the children that they were being disrespectful. Ms. DeGrave said "What did I do wrong?" She answered " You disrespect Mrs. Deiter and don't listen to her". She told them that Mrs. Deiter had done everything for them and that they were acting like bullies. She reminded them that Mrs. Deiter is their boss and Krista said that she did not care. Mrs. Chenier said she knew they talked about parents and other employees out loud in front of anyone present. On 4/6/21, Child A's Mother said that on 3/8/21 she received a picture text message of Child A in his pull-up from Krista Campbell and it looked very wet and sagging. This happened at about 3:45pm. Child A had been dropped off at about 8:30am that day by Child A's Relative and he was wearing a pull-up. At the center, there were only diapers for Child A. That is another reason she knew it had not been changed all day. He was just transitioning to pull-ups and she had not given the center any pull-ups yet. Krista Campbell told Child A's Mother that she had received a text from Mrs. Deiter at about 3:00pm asking her to check Child A's pull-up because she had forgotten to do this before she left. Krista Campbell forwarded Child A's Mother the message from Mrs. Deiter. Child A's Mother messaged Mrs. Deiter asking if Child A had used the potty chair that day. Mrs. Deiter said that she had put Child A on the potty earlier and that he had been dry all day. I asked Child A's Mother why a complaint was not filed about the diapering earlier. She said she did not know she could file a complaint and that at the end of March when she heard about the yelling going on at the center, she decided to file them both at once. Prior to 3/8/21, she had not had concerns about Child A's diapers not being changed often enough. 5 On 4/14/21, Mrs. Deiter said that they were trying to help potty train Child A. At about 10:30am on 3/8/21, she put him on the potty chair and his pull-up had been dry but he did not go on the potty chair. She then began to prepare lunch while Child A went outside with the other children. When they came in from outside, Child A's pull-up should have been checked by someone before lunch. She left the center at about 2:00pm and Child A was sleeping. A new staff member, Robin Marohnic, was working on the preschool side. Ms. Campbell had come in at 12:30p and Ms. Marohnic had worked 7:30am to 12:30pm. She was not sure if either of them had checked Child A's pull-up. Diapers and pull-ups were checked regularly at the center but she was concerned that Child A got missed. At about 3:00pm she messaged Krista asking her to check Child A's pull-up and let her know that he had been on the potty at 10:30am. At 3:30pm, Krista informed her that it had been soaked. On 4/21/21, Denisha Campbell said she got to work about 12:30 on 3/8/21 and worked on the preschool side. Diapers were usually checked before she got there. Child A was asleep when she arrived. They took the children outside and came in at about 1:00pm. Later that afternoon, Krista informed her that Child A's pull-up was full. On 4/21/21, Krista said she was working on the infant toddler side on 3/8/21. Mrs. Deiter should have changed Child A before he napped. She messaged her at 3:00pm asking her to check Child A's pull-up. She said that she had put him on the potty at 10:30am and he was dry. Child A's pull-up was full and she messaged Child A's Mother and let her know. This could have been against policy but she felt she should do it anyway. Krista said she felt diapers are usually changed at the center regularly. On 4/22/21, Child B's Mother said Child B, a 24 month-old female, had been at the center full-time for the last year and she had no concerns about her diaper getting changed regularly. On 4/22/21, Child C and Child D's Mother said Child C, a 24-month-old male, had been going to the center for about a year and a half and she had no concerns about his diapers not being changed on a regular basis. Child D, an 8-month-old male, had been going to the center for about 5 months and she had no concerns about his diapers not being changed regularly either. On 4/28/21, staff member Ms. DeGrave said she was working with Krista on the infant toddler side on 3/8/21. She remembered Krista telling her that she did not think Child A had been changed so she changed him. Ms. DeGrave said she felt 3/8/21 was a one- time situation and that she felt diapers were changed regularly at the cen
