2 On October 22, 2020, I received a telephone call from Brooke Mallory, program director/license designee. On October 22, 2020, Child A pushed open the door of the play room and went into the hallway of the facility. Child A walked to the front foyer of the building, back down the hallway and to the front office. Child A was observed outside of the office door by Ms. Mallory and child care staff members Teri Johnson and Chelsea Conley. Child A was under the care and supervision of Ms. Truember and Ms. Krug at the time of the incident. Ms. Johnson went to the play room to inform Ms. Truember and Ms. Krug that Child A was in the office. Ms. Truember and Ms. Krug were not aware that Child A had left the play room. On October 23, 2020, I completed an onsite inspection at the facility. I interviewed Ms. Mallory. Ms. Mallory was present in the office of the facility on October 22, 2020 when she heard Teri Johnson state, "Child A what are you doing?" Child A was in the care of the Wobbler Classroom staff; the classroom was engaged in activities in the play room of the facility. Child A was being supervised by Ms. Krug and Ms. Truember when she left the playroom. Ms. Krug was providing a restroom break to Ms. Conley, who was assigned to the classroom with Ms. Truember. The classroom had five toddler age children present at the time of the incident. Ms. Mallory spoke with Child A's Mother on October 22, 2020; she was not upset about the incident. Child A has a history of eloping, which the facility was not aware of. Ms. Mallory pulled video footage from the play room. She was able to see Child A push the door open and walk out of the area unnoticed by Ms. Truember and Ms. Krug. When Ms. Mallory spoke with Ms. Truember about the incident on October 22, 2020, Ms. Truember reported that Child A had attempted to leave the play room earlier in the day on October 22, 2020. On October 23, 2020, I viewed footage of the play room with Ms. Mallory. I observed Child A run around Ms. Truember and to the north set of double doors in the play room door. The doors are metal and lacking a middle support pole. Ms. Krug was standing facing away from the door and looking at Ms. Truember, who was facing the double doors. They were engaged in a conversation with each other while the remaining four children played. The video footage does not contain sound. Child A left the play room at approximately 2:47 pm the video footage time. The video footage time is off by about 15 minutes, but accurate in terms of the amount of time that elapsed. Child A was gone from the play room for approximately 90 seconds when Ms. Johnson enters the set of double doors which Child A left from. I viewed the situation again from the front foyer cameras. Child A is observed alone in the hallway when she walks to the front foyer area. She walked around in the foyer area and back down the hallway toward the office. Ms. Mallory does not believe Child A can reach the release bars on the front double doors. In Ms. Mallory's opinion, Child A would not be strong enough to push the panic bars to open the doors. At 2:48 pm and 38 seconds, Ms. Johnson is observed coming into the hallway in front of the office where Child A is standing. On October 23, 2020, I viewed the front foyer area with Ms. Mallory. The area contains a desk, two display cabinets, and one large, covered garbage can. I 3 observed one unopened bag of sidewalk salt and one plastic bin that contained sidewalk salt. The desk had one bottle of hand sanitizer. The garbage can had a garbage bag liner that was folder over the rim of the garbage can and loose down the sides of the garbage can. On October 23, 2020, I interviewed Ms. Truember. On October 22, 2020, she was providing care for five toddler age children with Ms. Johnson in the play room. Ms. Johnson was not present in the playroom when Child A left the area unattended. Ms. Krug stepped in to provide a restroom break for Ms. Johnson. Ms. Truember is not sure how Child A

On October 15, 2020, 1 received an incident report from Brooke Mallory, program director/licensee designee. On October 14, 2020, Child A's Mother stopped in the front office of the facility and requested to speak with Ms. Mallory. Ms. Mallory was not present at the facility on the morning of October 14 2020. Child A's Mother stated to present individuals which included and Volunteer, that a child care staff member had een "laying her hands on him (Child A). Child A's Mother directly told that she was never to be alone with her children again. Ms. Mallory was able to speak with Child A's Mother in the afternoon of October 14, 2020. Child A's Mother stated to Ms. Mallory that Child A told her on Monday and Tuesday night October 12 2020 and October 13, 2020 that had been hitting him on On October 16, 2020, l interviewed Ms. Mallory Own itAto Rerl , 2020 Ms. Mallory received a text message from Ms. Mallory was out of the office at the time. Child A's Mother had stopped in the office and was visually upset; she was teary and shaking. Child A reported to his mother that1 put her hands on him. Child A's Mother never wants a one with er children again. Ms. Mallory spoke with on the afternoon of October 14, 2020. She asked if s e put er ands-on Child A. denied the allegation. Ms. Mauory spoke with Child A's Mother on the ernoon of October 14, Pc 2020. On Monday October 12, 2020, Child A told his mother that 1 3 hit him on At first Child A's Mother blew him off. On Tuesday o er , 2020, Child A again told his mother that hit him on The child was inconsolable. Child A's Mother reachedW ouo Hailev Smith. child care staff member, via text message and was on a verbal tirade, about the incident. A copy of the message was supplied by Ms. Mallory. The message states that Child A's Mother is upset over the allegations between Child A and Child A's 3 Mother used threatening language toward and that she intended to address the issue with Ms. Mallory. ChildlsWoMother believes Child A was stating the truth about hitting him on Child A's Mother stated that I takes tmild A in the bathroom to hit him. Child A demonstrated by hitting himself in the Child A's Mother requested that Ms. Mallory speak with Child A in her office. Child A's Mother brought Child A to the office and began asking him questions in front of Ms. Mallory. Child A said no one hit him. Child A's Mother stopped questioning Child A because he was getting overwhelmed. Ms. Mallory asked Child A's Mother if she had observed any marks on Child A. Child A's Mother responded "No." Cameras are present in the classrooms. , was present from approximately 12:00 pm until 6:00 pm on October 12, 2020 and October 13, 2020. Ms. Mallory and Kelli Ottinger, child care staff member, have begun to review film footage. has not had any previous complaints regarding use of physical discipline on child care children. Ms. Mallory has had reports form child care staff members this week that Child A was observed being untruthful in the classroom. He stated another child care child was swearing at him.
