On April 25, 2023, I interviewed Child A and Child B's Mother. Child A's Mother is not provided with everything that Child A eats on a daily basis. The facility uses the Brightwheel application and will only log one or two items that Child A eats. Child A's Mother provided screen shots of the Brightwheel application for Child A which showed her food intake for April 17, 2023, and April 18, 2023 On April 26, 2023, I completed an unannounced onsite inspection at the facility. I interviewed Sarah Brooks, program director/ licensee designee. The facility uses the Brightwheel application for parents to obtain information on their child's day. The facility logs what children eat during the meals. Parents provide the lunch and breakfast for children and the facility provides the snacks. Child A is 2.5 years of age and is over the age of being a toddler. The facility logs food for parents of all children even though is not required per the child care center licensing rules. On April 26, 2023, I interviewed CCSM 1, CCSM 2 and CCSM 3, the facility logs the food children eat at the facility on Brightwheel. CCSM 1 and CCSM 2 identified that Child A spends a lot of time playing with her food and will not always eat all of the food provided.

On April 25, 2023, I interviewed Child A and Child B's Mother. The facility allows school age children to play on unsafe playground equipment that is not inspected. Child B broke his arm on the equipment last summer. On April 26, 2023, I completed an unannounced onsite inspection at the facility. I interviewed Sarah Brooks, program director/ licensee designee. Ms. Brooks submitted the incident report to the department regarding Child B on June 13, 2022. Child A and Child B's Mother signed an enrollment form for Child B on September 23, 2021, which included a statement for the playground equipment used by school age children. The equipment is at a building operating in a school building. On April 26, 2023, I observed the playground equipment and did not observe any broken equipment. Review of system records indicate that I spoke with Child A and B's Mother on June 13, 2022, regarding Child B and no concerns were expressed.

(12) School-age centers operating in school buildings approved by the Michigan department of education are exempt from subrule (11) of this rule, provided the licensee informs parents, in writing at the time of enrollment, if the center plans to use a public school's outdoor play area and equipment that do not comply with subrule (11) of this rule.
On April 25, 2023, I interviewed Child A and Child B's Mother. On April 10, 2023, Child A's Mother brought Child A a container of Lactose milk. The milk was not opened until April 13, 2023, at 11:30 am. The new container of milk is good for seven days once opened. Child A's Mother stated that she did not bring milk for 5 Child A in another container. Child A is not getting enough milk based on what is being sent home at the end of the week. On April 25, 2023, Child A's Mother sent a screen shot of a conversation between herself and Ms. Brooks regarding some confusion regarding the milk dating. Ms. Brooks apologized regarding the confusion and said she would speak with the child care staff. On April 26, 2023, I completed an unannounced onsite inspection at the facility. I interviewed Sarah Brooks, program director/ licensee designee. Child A was not served the milk until April 13, 2023, because the center still had milk for Child A that was still good. Ms. Brooks reported ongoing issues with Child A's Mother bringing back milk that was previously sent home from the facility that exceeded the 7 days. Child A's Mother has been spoken to on multiple occasions regarding food and beverage service for Child A. There was confusion regarding the dating on the milk this month. Ms. Brooks addressed the confusion with the child care staff. Child A's Mother initially supplied a doctors note regarding Child A not having any milk at the facility. On April 26, 2023, I provided technical assistance to Ms. Brooks regarding adding an additional statement to her enrollment policies regarding parents are required to supply food for children with dietary needs. The current statement that parents sign states that children with dietary needs should speak to the director. On April 26, 2023, I interviewed CCSM 1, CCSM 2, and CCSM 3 regarding food service for Child A. All staff identified that Child A was served milk when Child A's Mother supplied the milk to the facility. On April 28, 2023, I advised Serrenna Sengstock, Licensee, not to allow opened containers of milk that are returned to the child's parent to be returned to the facility, even if the seven days are not up. Commercial containers of milk provided to the facility for a multi-day use must be unopened.

On December 1, 2022, the facility did not provide appropriate care and supervision to Child A, when they did not inform Child A's Father or Child A's Mother that he was not present at afterschool programming as scheduled. Child A did not arrive at the after-school meeting place for middle school children. Kelli Ottinger brought the school age children back to the child care facility and did not inform office staff that Child A was not present. Child A rode the bus home from school and was alone for approximately two hours.

On August 3, 2022, I received a telephone call from Serrenna Sengstock, licensee, to report that Child A was left unattended in the Prepper Classroom for a total of four 2 minutes and three seconds. Child A was being cared for by Teri Boyer, child care staff member, and Katriana Curtoys, child care staff member. Child A was found crying at the doorway of the classroom by Betty Binder, office staff member. Child A was not injured. On August 8, 2022, I interviewed Child A's Mother. The child care staff counted all the children before going outside. Child A left the line to go to the bathroom and was left alone in the classroom for about four minutes. An office worker heard Child A crying and took her outside. Child A was not injured and remains enrolled at the facility. Child A's Mother does not have any concerns with the care and supervision provided by the child care facility. On August 8, 2022, I completed an unannounced onsite inspection with Tania Allard, child care licensing consultant. I interviewed Sarah Brooks, licensee designee/program director. Ms. Brooks was not present at the facility at the time of the incident. Ms. Brooks will be working with all child care staff members to start using the name to face check option through the Bright Wheel application. It allows child care staff members to physically check each child out of the classroom on the application to another location, such as outside or the gymnasium. On August 8, 2022, I interviewed Teri Boyer. Ms. Boyer was working in the Prepper Classroom with Ms. Curtoys and eight child care children. The children were ages 2.5-3 years. Ms. Boyer completed a head count and counted Child A first. Ms. Boyer was at the front of the line and Ms. Curtoys was at the back of the line. Ms. Boyer did not complete a second head count in the hallway or when she got outside with the child care children. Ms. Boyer did not realize Child A was not present until Ms. Binder brought her outside approximately four minutes after they came outside. Child A was not injured and remained enrolled at the facility. When Ms. Boyer viewed the video, she observed Child A walk behind her after she counted child care children and into the bathroom. Ms. Boyer will count children more consistently and use the check in/out feature on Bright Wheel. Ms. Boyer spoke with Child A's Father the day after the incident. Child A's Father stated that Child A's Mother and Child A's Father laughed about the incident and said they always tell Child A to go into the potty when she needs to use the bathroom. Ms. Boyer does not have any concerns with the care and supervision provided at the child care center or by Ms. Curtoys. On August 8, 2022, I reviewed the video footage from August 3, 2022, with Ms. Sengstock. The video shows Child A leaving the area where child care children were lining up to go outside and walking into the bathroom. Ms. Curtoys and Ms. Binder left the classroom with seven child care children. Child A exits the bathroom, washes her hands, and goes to the door of the classroom. Child A stands at the door of the classroom and Ms. Binder comes to the doorway. The total time Child A was alone in the classroom was approximately four minutes. On August 8, 2022, I interviewed Ms. Curtoys. Ms. Curtoys was working in the Prepper Classroom with Ms. Boyer and 8 children. The classroom as going outside, 3 and Child A got out of line. Both Ms. Curtoys and Ms. Boyer completed a head count inside the classroom but did not count again in the hallway or when they arrived outside. Ms. Binder found Child A and brought her outside. Ms. Curtoys was not aware that Child A was not with the group. Ms. Curtoys does not have any concerns with the care and supervision provided by the child care facility or Ms. Boyer. In the future Ms. Curtoys will recount children when transitioning.

R 400.8112 · R 400.8112 Comprehensive background check; fingerprinting. (1) Pursuant to section 5n of the act, MCL 722.115n, before an individual has unsupervised contact with children, the department shall determine the individual's eligibility to be any of the following: (a) A licensee.
