On November 4, 2022, I received a telephone call from Jeff Guza, licensee designee/program director, regarding Child A slipping on a toy. Child A fell forward into a cubby, hitting her head on the corner of the cubby. Child A received stitches on her forehead due to her injury. In the incident report received from Mr. Guza on November 4, 2022, it noted that attempts were made to contact Child A's parent(s) at 10: 44 am and 10: 45 am. No answer was received. At 10:46 am the decision was made for Huron Learning Center staff to transport Child A to the emergency room. Child A was transported in a school owned van in an age- 2 appropriate child safety seat. Child A's Mother arrived at the hospital following Child A arriving at the emergency room. On November 4, 2022, a review of submitted documentation from the child care facility indicate the use of 15-48 passenger vehicles for transportation of child care children. The Self-Certification of Transportation Provisions for Centers was last submitted in August of 2022. On November 4, 2022, Mr. Guza submitted enrollment documents for the facility including a signed statement by Child A's Mother granting permission for transportation in the event of an emergency. On November 4, 2022, I interviewed Child A's Mother. Child A's Mother does not have any concerns with the care provided to Child A by the facility. Child A's Mother met Child A and Ms. Christina (Christina Kosinski, child care staff member) at the hospital. Child A is doing well following her injury. Child A's Mother recalled signing enrollment paper work for Child A at the start of the school year. She can not recall if the paper work included permission for transportation. On November 9, 2022, I completed an onsite inspection at the child care facility. I interviewed Mr. Guza. Mr. Guza and I viewed the van used to transport Child A to the hospital, which was approximately one mile away. The van is a Ford 7 passenger vehicle with approximately 13,000 miles. The van had emergency equipment in the trunk and a five-point harness child safety seat installed in the rear of the vehicle. The van was driven by Rebecca Schumacher, assistant principal. Ms. Schumacher has a valid chauffeurs license and completed a drivers record check at the secretary of state on November 9, 2022. The check was not completed at the time of the incident. A vehicle inspection report was not on file at the facility. Mr. Guza was not aware of the transportation requirements and will make arrangements to complete the required inspection. Ms. Schumacher is not an employee of the child care facility and was not alone with Child A. I reviewed with Mr. Guza transportation requirements and child care staff member requirements during the onsite inspection. On November 9, 2022, I interviewed Ms. Kosinski. Ms. Kosinski was present when Child A injured her head on November 4, 2022 and observed her fall. Ms. Kosinski applied first aid to Child A and alerted the school medical response team following the injury. Attempts were made to contact Child A's parent(s) and grandparents prior to transporting her to the hospital. Ms. Kosinski rode with Child A to the hospital and remained with her during treatment at the hospital, including after the arrival of Child A's Mother. Child A was not left alone with any non-child care center staff. 3
