On 8/17/23, I interviewed Lee Carpenter, certified playground inspector. Mr. Carpenter stated that he did complete the playground inspection at Ready, Set, Grow child care center. He recalled inspecting the dome climber and he approved it for children preschool age. Mr. Carpenter revisited the center because Ms. Colberg added the dome climber after his inspection and requested that he come back to inspect the new climber. I reviewed the playground inspection, and it was completed on 4/18/23. Mr. Carpenter approved the dome climber for children three to 10 years of age. There 2 was enough surfacing for the playground at the time of the playground inspection. Ms. Colberg provided me with an invoice for surfacing that was added to the playground. It was dated 5/03/23. I interviewed Child A's Mother. She did not have any concerns about Child A's injury and how it occurred. She was contacted in a timely manner and was able to pick up Child A to have her examined. On 8/23/23, I made an on-site inspection. I interviewed Ms. Colberg and observed the playground. Ms. Colberg stated that Child A was climbing up the side of the climber and was almost to the top. She lost her footing and fell to the ground on and landed on her left arm. The climber that Ms. Colberg showed me that Child A fell from was the climber in the playground inspection report approved for children three years of age through 10 years.
