On April 20, 2023, Program director Beth Blanchard submitted an incident report indicating that on April 19, 2023, Child Care Staff Member 2 held Child A (male, age 19 months) by the hands and spun him around causing Child A's elbow to be partially dislocated resulting in Nursemaid elbow. The injury required medical attention to put this elbow back in place. During the onsite inspection on April 24, 2023, Ms. Blanchard stated that Child Care Staff Member 2 is a "student 2 teacher" from the University of Michigan who is no longer working at the center due to the end of the semester. Ms. Blanchard stated that all child care staff members including "student teachers" are trained on the appropriate to way to pick up children. During the training, the dangers of picking children up by the hands are explained. Ms. Blanchard stated that Child Care Staff Member 2 stated that she was not thinking when she picked Child A up by the hands and swung him around. Child Care Staff Member 2 stated that she was trying to play with Child A. Child Care Staff Member 1 stated that she was working in the classroom with "student teacher" Child Care Staff Member 2 on April 19, 2023. Child Care Staff Member 1 stated that they were outside when she heard Child A crying. She did not see what happened, but Child Care Staff Member 2 told her that she was spinning Child A around and he might have gotten dizzy. Child A's Father walked in as Child A was crying. Child Care Staff Member 1 stated that she told Child A's Father that Child Care Staff Member 2 was spinning Child A around and she thought that Child A may be dizzy. Child Care Staff Member 2 left for the day at 5:15 PM. After all the children were picked up for the day Child Care Staff Member 1 closed the classroom and walked up to the front of the center. Child A's Father was still in the lobby trying to put Child A in the stroller so they could walk home but Child A was still crying. Child A's Father called Child A's Mother to come to the center with the car so that they could take Child A to the hospital. While waiting for Child A's Mother to arrive Child Care Staff Member 1 stated that she offered Child A snacks to help him calm down. Child A stopped crying, but Child Care Staff Member 1 noticed that Child A was not moving his left arm. Child A's Parents took Child A to the ER at the University of Michigan Hospital. The following day Child A returned to the center and Child A's Mother reported that Child A had Nursemaid elbow and his elbow had to be put back in place. Child A was perfectly fine and using his arm as normal the day after the incident. Child Care Staff Member 1 informed Child Care Staff Member 2 of Child A's injury and reminded Child Care Staff Member 2 that she must hold children under their arms when she picks them up. Child Care Staff Member 2 stated that she was really sorry and that this was the first time that she did that. Child Care Staff Member 2 stated that she feels "really bad", and she loves Child A. Child Care Staff Member 1 stated that Child Care Staff Member 2's college semester is over and she will not be returning to the center. Child Care staff Member 1 stated that all child care staff members are trained on the proper way to pick up children to prevent Nursemaid elbow so she is not sure why Child Care Staff Member 2 picked Child A up by the hands. I attempted to contact Child Care Staff Member 2 who is a student teacher who no longer works at the center by phone and by email but did not receive a return call or email response. 3

Program director, Beth Blanchard, reported that on February 23, 2021 Child A (male, age 3 years) was left unattended in the stairwell by staff member, Mary McCarthy, who was substituting in the Oak preschool room when she was taking a group of eight children outside to the playground. Between two and five minutes later, staff member, Courtney Rouse, who was also working in the Oak preschool room, was coming down the stairs with two children when she saw Child A standing near the closed door that leads to the playground. Ms. Rouse took Child A outside to the playground and told Ms. McCarthy that she had left Child A in the stairwell. Ms. Rouse then reported the incident to Ms. Blanchard. Ms. Blanchard stated that she spoke with Ms. McCarthy about the incident and she stated that she doesn't know how it happened. Ms. McCarthy said that she counted the children as they were heading out the door to the playground and she thought that she counted eight children, but she must have counted someone twice. Ms. Blanchard reminded Ms. McCarthy that she is supposed to count the children in the 2 stairwell and again when they reach the playground. Ms. Blanchard reported the incident to Child A's Mother and Father on February 23, 2021. They appreciated being told and were relieved that he wasn't hurt. Ms. Blanchard stated that Child A remained in the stairwell instead of going out the door to the playground because children are taught that opening doors is an adult's job. Ms. Blanchard stated that this type of incident has never happened before at the center. Staff member, Mary McCarthy, has been employed by the center for 2 years. She 1/2 stated that she is not assigned to a particular classroom, as she moves around to help where she is needed. Ms. McCarthy stated that on February 23, 2021 she was working in the Oak preschool room with Courtney Rouse. There were 10 children in the classroom. Ms. McCarthy and Ms. Rouse were assisting the children with getting ready to go outside to play. Ms. McCarthy took eight of the ten children outside first while Ms. Rouse was left with the remaining two children. Ms. McCarthy stated that one of the children was assigned to be the door holder so that child held the door at the top of the steps while she led the other children down the steps. When the children made it downstairs, Ms. McCarthy held the door open that leads to the playground while the children exited the building. Ms. McCarthy stated that she was counting the children as they were going out the door and she thought that she counted eight, but Child A must have been lingering on the staircase because she didn't see him. After Ms. McCarthy had been outside for about two minutes, Ms. Rouse came outside with Child A and the other two children. Ms. Rouse told Ms. McCarthy that Child A was left in the stairwell. Courtney Rouse has been employed by the center for one year. She is assigned to the Oak preschool classroom. On February 23, 2021 she was working with Ms. McCarthy because the other staff member that normally works in the classroom was reassigned to another assignment for the day. Ms. Rouse stated that the 10 children in the classroom had just finished snack when they were getting ready to go outside. Ms. McCarthy took the eight children that were finished getting dressed outside first while she remained with the other two children who were still getting dressed. Ms. Rouse stated that about five minutes after Ms. McCarthy left the room with the eight children, she left the room with the other two children. As Ms. Rouse was going down the steps, she heard noises and looked down and saw Child A. Child A was standing by the door that leads to the playground rubbing his hands on the door. Child A told Ms. Rouse that he wanted to go outside. Ms. Rouse told Child A that she was coming downstairs and she would take him outside. Ms. Rouse stated that Child A was not upset or crying.
