On May 5, 2021, I completed an unannounced on-site inspection. I spoke to program director, Diane Wasielewski, and individually interviewed child care staff, Laurie Shore, Teresa Crooks, Monica Higman, Carol Stacy, and Tasha Drinkard. Ms. Wasielewski and child care staff denied that the toddler room was out of ratio on May 3, 2021. All child care staff stated that the child care center always maintain proper ratios. On May 5, 2021, I completed an unannounced on-site inspection. I spoke to program director, Diane Wasielewski and individually interviewed child care staff, Laurie Shore, Teresa Crooks, Monica Higman, Carol Stacy, and Tasha Drinkard. Ms. Wasielewski denied that mouse traps were ever laying around accessible or in sight of children or others. She also denied that the center is ever dirty. She did state that there is a D-Con mouse bait container placed in a locked cabinet in a child care room and another in the laundry room, which is kept locked. All child care staff denied ever observing mouse traps laying around or being accessible or in sight of children or others. I observed the two D-Con mouse bait containers to be in locked locations inaccessible and out of sight of children and others. The child care center was also observed to be maintained in clean and sanitary conditions. On May 12, 20221, I spoke to Child A&B's Mother and Child C&D's Mother. They all denied that they have ever observed any mouse traps at the child center. They also all denied observing the child care center to be dirty. 4
