    On 03/29/2023 1 received a complaint regarding a significant injury to Child A, a The report stated spoke with i"S o- ther on 03/28/202. Child A's Mother said she had just remembered that three days ago, Child A had fallen out of her rocker while Child A's Mother was in the kitchen. Child A's Mother explained she was washing dishes and Child A was next to her in her rocker and Child A was moving around, fell out of the rocker and hit her head. On 03/29/2023 1 spoke with . Plans were made to meet at a local establishment first to conduct joint phone calls then to make an unannounced onsite visit to the child care home afterwards. On 03/2912023 and I spoke with She explained when Child A was first examined on "J2023, she had blood under her scalp, but the CT scans showed no fractures. However, the radiologist read the scan and stated there was a skull fracture on Child A. was unable to provide the name and contact information on the radiologist. On 03/29/2023 and I called the child will look at Child A's informationRanqreuorn Rommoun"ica n. On 03/29/2023 and I made an unannounced visit to the child care home. Ms. Awad She said Child A attended child care on Monday, 03/27/2023 and appeared normal with no injuries observed. Child A did not attendchild care on Tuesday, 03/28/2023, but returned this morning. When Child A's Mother dropped off today, Child A's Mother said Child A fell on the floor approximately two days ago. Child A's Mother said Child A cried for a moment and appeared fine, but Child A's Mother noticed a bump on Child A yesterday, 03/28/2023 and took Child A to the doctors. Ms. Awad told Child A's Mother she 3 should never wait two days to check on a head injury. Ms. Awad stated Child A did not fall or hit her head in any capacity while she was in care on Monday 03/27/2023. Ms. Awad has four child care staff members, including her CCSM 1,CCSM 2 and CCSM 3 were interviewed along with " Oon, reported Child A being harmed, falling, or bumping her head in any capacity on Monday 0312712023. No one reported any concerns regarding discipline or the care and supervision to any of the children. While onsite, and I stepped outside to take a phone call from the were interviewed, confirmed a hematoma was a result of e "SU fracture which resulted rom a drop, fall or some sort of blow to the head. The fracture likely happened first and then the swelling and bleeding occurred as a result. It is difficult to say how long it would occur for the bump to form, but swelling can occur for a day or two and might not be noticeable if the child has a full head of hair. Child A does have thick, dark hair. It could be that the bump was not noticeable for a few days, and it could be missed as the has seen this occur before, informed them that s Mother disclosed Child A had fallen out of her rocker three days ago, they noted some concerns with how a R can fell out of a rocker but will plan follow up with Child A's Mother for aaddii tional information. and I returned to the child care home. Child A was "OseRrrrNve Roe in a bouncy seat and appeared well dressed and content. On 0313012023, 1 received an email from ,. She said skull fractures cannot be dated as they do not show signs of healing on x-rays, however with the scalp swelling present, the injury likely occurred sometime "in the prior several days/weeks". On 03/31/2023, I received an email from stating she is no longer investigating these allegations. On 05/02/2023 I spoke with Child A's Mother. She said Child A fell out of a rocker and was taken to the hospital for a head injury, but this happened under her care. She said Child A did not fall or receive any injuries while in the care of Ms. Awad. She said she has no concerns for Child A attending Ms. Awad's child care and feels she does a great job keeping the children safe and happy. She said Child A "loves" Ms. Awad and enjoys going to her child care. She said Child A has never received any injuries while in Ms. Awad's care. CI
