CCSM 1, CCSM 2 and CCM 3 did not provide appropriate care and supervision of Child A when Child A was left unattended outside for approximately three minutes.

On April 8, 2021 I spoke to Child A's Mother. Child A's Mother stated that her daughter was left alone in the classroom when the rest of the children in the classroom went outside. Based on the information she could gather it appears that she was alone for a few minutes. Child A's Mother stated that Child A told her that she was crying when she was alone in the classroom. On April 13, 2021 I made an unannounced onsite inspection to the child care center. I interviewed the Program Director Sonja Knighton. Ms. Knighton stated that she was aware of the incident and had spoken to the child care staff members about the 2 incident. Child A was left in the downstairs classroom when the other children were transitioning outside. She does not know why Child A was not in the line with the other children. I asked what protocols the center had in place to prevent children from being left during transitions. Ms. Knighton stated that the child care staff members are supposed to do a head count between transitions, but the child care staff did not follow protocol during this incident. There was a flex staff member and a substitute working in the classroom. When they reached the top of the stairs and went outside they closed the door. A child care staff member heard Child A crying inside and opened the door. Since the child care staff members were still at the top of the stairs, it appears Child A was alone in the classroom 1 to 2 minutes. 1/2 On April 13, 2021 I interviewed child care staff member Alon Samuel. Mr. Samuel stated he has worked at the center approximately four months. I made Mr. Samuel aware of the allegations in the complaint. Mr. Samuel was aware of the incident that occurred on April 7, 2021. He said he and Ms. Isabell were transitioning the children from naptime to outdoor time. He had the children pack up their gear and head outside. They walked up the stairs and headed outside. When they got outside Child A was pushing at the door and crying. Isabell heard her and opened the door. I asked how long they had been outside when they heard Child A crying, he stated they were outside one or two minutes. Mr. Samuel went on to state that the protocol when transitioning children is to line them up and count them before leaving. They are also supposed to periodically count the children while they are outside. He had the line going but did not count the children before going outside. Mr. Samuel stated that he was the senior staff member in the classroom, and he was responsible for counting the children. On April 13, 2021 I conducted an exit conference with the Program Director Sonja Knighton. Ms. Knighton stated that she is going to emphasize the importance of following the protocols they have in place with the staff members to assure this does not continue to happen. On May 25, 2021 I called Child A's Mother and asked if she had any concerns regarding the centers handling of the incident. Child A's Mother stated that overall, she is satisfied with the way things were handled. She has been assured that the child care staff are following all of the transition protocols. 3
