On March 30, 2023, I completed an unannounced on-site inspection. I individually interviewed Program Director, LaTanya Burrell and Child Care Staff Member 1 and Child Care Staff Member 3. Ms. Burrell stated that Child A (male, ) has had 14 separate incidents of biting and recently injured a child. Ms. Burrell was promoted to program director February 2023, and has been working with human resources to resolve this matter. Ms. Burrell stated that she felt the previous program director did not attempt to resolve the issue when it was brought to her attention. She stated that when child care staff members provided incident reports and discussed the biting behavior with the regional , she told child care staff members that it was age appropridatier ebcethoar"vaionrMeys uld provide him one on one attention, ■ provided biting rings and a pacifier for Child A, but this did not decrease his biting incidents. Ms. Burrell stated that after meeting with human resources last week she learned that the child care center had a policy for removal from the program for biting and harmful behaviors that cannot be resolved after develo in an individualized behavior plan. Ms. Burrell and human resources will meet with to discuss strategies implemented and dismissal from the program due o not being able to meet his needs. She stated that the child care staff members have attempted to watch him closely but there are not enough child care staff members to provide him one on one care. Child Care Staff Member 1 stated that Child A has had multiple incidents of biting other children. While she was checking a child in yesterday Child A bit another child on the back breaking the skin causing bleeding. She stated that child care staff members discussed the concern with the previous director> and she felt the concerns were not addressed, told child care staff members that Child A's behavior was age appropriate and they were not doing enough to provide him the support and one on one attention needed. She stated that on one occasion ■ directed her to revise an incident report to state that Child A's biting was age appropriate. Care Staff Member 1 stated that she attempted to watch him closely, but he bites quickly before he can be stopped. Child Care Staff Member 2 stated that that Child A has had multiple incidents of biting other children, tells child care staff members that Child A's behavior is age appropriate and they are not doing enough to provide him the support and one on one attention needed. Child Care Staff Member 2 stated that child care staff members have attempted to stop him from biting but it continues. on-site inspection today. l told that 1 completed an on-site inspection on March her 30, 2023, on the same allegations and would not be able to accompany her today due to another commitment. On April 19, 2023, I spoke to Child B's Mother. She stated that she is upset, as Child B was bitten twice, once breaking the skin causing injury. She asked to have Child B moved to another classroom. However, after Child B returned to the child care center after a two week vacation, Child B's Mother was informed that the child who bite Child B no longer attends the center. On April 19, 2023, 1 spoke to Child C's Father. He stated that Child C was bitten on more than one occasion. He stated that although he understands that biting is age appropriate, he was concerned about the multiple incidents. On April 19, 2023, I spoke to Child D's Mother. She stated that she did not have any concerns regarding the care or supervision provided. *OnA ril 23,2023. l articipated in a telephone interview with stated that her employment was terming e u no ue o e t incidentsof Child A. She is no longer the licensee designee or regional director of the child care center. stated that the child care center did not have a biting policy but had a harmful behavior policy that she developed. She stated that she asked child care staff members to not give Child A preferential treatment

On March 30, 2023, I completed an unannounced on-site inspection. I individually interviewed Program Director, LaTanya Burrell. Ms. Burrell denied the center operates out of ratio. Ms. Burrell stated that she believes the allegations came from a child care staff member who she terminated last week. Ms. Burrell stated that she works in the classrooms as a child care staff member to ensure that ratio is maintained. She has hired new child care staff members that will begin employment next week. Child Care Staff Member 1 and Child Care Staff Member 2 denied the child care center operates out of ratio. During my on-site inspection I reviewed children and staff daily attendance records and schedules for children and child care staff members from May 2023. I did not observe any classroom to be out of ratio on any days. During my on-site inspection I observed six infant and toddlers ages birth to 30 months present with two child care 5 staff members. I observed five preschool children ages 3 to 5 years old present with one child care staff member. On April 19, 2023, I completed a second on-site inspection to observe classroom ratios. I observed seven infant and toddlers ages birth to 30 months present with three child care staff members. I observed eight preschool children ages 3 to 5 years old present with one child care staff member. On April 19, 2023, I spoke to Child B's Mother Child C's Father and Child D's Mother. They all stated they never observed the center to operate out of ratio.
