    On May 8, 2023, I spoke to the Complainant. The Complainant stated that scratches were observed on Child A (male, 29 months) face on April 24, 2023, May 2, 2023, and May 5, 2023. The Complainant stated that an incident report was received for the April 24, 2023, incident. The Complainant stated that an incident report was not received on May 2, 2023 and May 5, 2023, as scratches were observed on his face when he arrived home. I asked the Complainant if the scratches were observed during pick up at the child care center. The Complainant reported not seeing scratches on Child A's face at pick up, and not seeing them until after arriving home on May 2, 2023, and May 5, 2023. Complainant reported speaking to the Lead Child Care Staff Member 1 on May 3, 2023, regarding the 2 scratches and not receiving an incident report. Lead Child Care Staff member 1 told the Complainant that Child A was not observed to have scratches on May 2, 2023. Complainant stated on May 5, 2023, when the Complainant observed scratches on Child A's face again after arriving home the Complainant disenrolled Child A from the child care center. The Complainant stated that Child A was enrolled at the child care center for one month and the Complainant was concerned about the supervision being provided and not receiving incident reports. On May 12,2023, I completed an unannounced on-site inspection. I individually interviewed Program Director Brandy Coffey, Lead Child Care Staff Member 1, Child Care Staff Member 2, and Child Care Staff Member 3. Ms. Coffey stated that an incident report was provided to Child A's Parents on April 24, 2023, as Child A sustained scratches on his face. Ms. Coffey also stated that Lead Child Care Staff Member 1 denied that Child A sustained any scratches at the child care center on May 2, 2023 or May 5, 2023. Ms. Coffey stated that the policy is to provide parents a written incident report at pick up for minor incidents and injuries that require minimal first aid. Parents are notified immediately via telephone for serious injuries and incidents. The lead child care staff member and director sign the form. The incident report is given to the parent and a copy is kept on file at the center. Ms. Coffey provided me a copy of the policy. I also reviewed a copy of the incident report for Child A and incidents reports for Child A's classroom. Lead Child Care Staff Member 1 denied observing scratches on Child A on May 2, 2023, or May 5, 2023. She stated that on April 24, 2023, Child A was scratched by another child while playing. An incident report was completed and provided to both parents. Lead Child Care Staff Member 1 stated that she watched Child A closely after he was scratched on April 24, 2023, and Child A's Father expressed concern. She stated that Child A's Father contacted her on May 3, 2023, regarding scratches he observed on Child A on May 2, 2023, after arriving home. Lead Child Care Staff Member 1 told him that scratches were not observed on Child A by any of the Child Care Staff Members. She also stated that Child A's Father did not question any of the Child Care Staff Members on May 2, 2023, or May 5, 2023, when he picked Child A up. Lead Child Care Staff Member 1 stated that parents are provided with a written incident report at pick up for minor incidents and injuries that require minimal first aid.
