On July 29, 2021, I completed an unannounced on-site insaection at the child care center with care staff members, Soumala Azzat and Dania Zaidan. Ms. Azzat stated that the licensee designee and program director, Majeda Issa would not be in today. and I called Ms. Issa and conducted a telephone interview. Ms. Issa stated she has 7 not been present at the child care center since July 21, 2021, due to having a r ical procedure com leted. She until July 30, 2021, due to i Ms. Issa stated on July 26, 2021, she received a telephone call from Child A's Mother. Child A's Mother stated that she was upset, as she thought Child A was left alone at the child care center. Child A's Mother stated that she observed him in his crib crying, and she did not see any child care staff members. Child A's Mother told her that she rang the doorbell for approximately ten minutes before a child care staff member came to the door, Child A's Mother entered through the main door into the vestibule. The vestibule door is kept locked and is all glass, which allows you to see inside the child care center. Ms. Issa denied that children are ever left unattended or unsupervised. She stated that she spoke with Ms. Soumaia Azzat, Child A's primary caregiver after Child A's Mother left the child care center. Ms. Azzat informed her that Child A's Mother arrived during naptime to pick him up. Child A was not asleep and was in his crib crying. However, Ms. Azzat stated she was changing another child's diaper and could not pick him up. The music was playing in both classrooms, and she did not immediately hear the doorbell. Child care staff member, Dania Zaidan let Child A's Mother in. Child A's Mother disenrolled him from the child care center. Ms. Issa stated that the ringer for the doorbell was located the hallway. However, immediately following this incident ringers were added inside the two classrooms. I observed the ringers inside the classrooms during my on-site inspection. Ms. Issa denied there were any previous incidents of child care staff members not hearing the doorbell. Ms. Azzat stated that she was the primary caregiver for Child A. She denied that she left Child A unattended or unsupervised. She stated that on July 26, 2021, Child A's Mother arrived during naptime to pick him up. Child A was not asleep when she arrived and was in his crib crying. Ms. Azzat stated she could not pick him up, as she was changing another child's diaper. Music is played in the two classrooms during naptime, and she did not immediately hear the doorbell. Child care staff member, Dania Zaidan let Child A's Mother in, as she was changing another child's diaper. Ms. Azzat stated that this was the first time she experienced not immediately hearing the doorbell, and she apologized to Child A's Mother. Child A's Mother took Child A and left the center. Ms. Zaidan denied that Child A was left unattended or unsupervised. Ms. Zaidan is a child care staff member in the Pre-K classroom. She stated that on July 26, 2021, Child A's Mother arrived during naptime to pick him up. When she heard the doorbell ring she went to the door and let Child A's Mother in, as she observed Ms. Azzat in her classroom changing a child's diaper. The infant classroom and Pre-K classroom are adjacent to each other with a two-way glass, which allow you to see into both classrooms. Ms. Zaidan stated Child A's Mother appeared upset when she entered, as she told her that she rang the doorbell for approximately five minutes. Ms. Zaidan apologized and explained that they did not hear the door bell, as the music was 3 playing loudly. Ms. Zaidan stated that this was the first time she experienced not immediately hearing the doorbell. On July 29, 2021, I completed a visit to Child A's home with _ and I interviewed Child A's Mother. Child A's Mother stated that Child egan attending the child care center part time on July 7, 2021. On July 26, 2021, Child A's Mother arrived at the child care center at approximately 1:30 p
