R 400.1911 · R 400.1911 Care; supervision; children. (1) A licensee shall ensure appropriate care and supervision of children at all times.
On 09/19/2022 Jamie Lewis, licensee, contacted Stephanie Shell, licensing consultant, to report an injury to Child A, female age 20 months. She reported that on 09/15/2022, Child A woke up from her nap and she went to pick her up out of the pack-n-play by having her grab the licensee's thumbs and lifting her up. This caused Child A to have an injury to her elbow. She called her parent's, and Child A received medical treatment. On 09/20/2022, Ms. Lewis emailed the required injury/accident reporting form to Ms. Shell. 2 Ms. Shell stated she also spoke with Child A's Mother about the injury. I did not receive any information on that contact. On 09/23/2022 I made telephone contact with Child A's Mother. She stated that Child A was lifted out of her pack-n-play by the licensee having Child A's arms extended and then grab the licensee's thumbs to lift her out. This caused Child A's elbow to be displaced, diagnosed as "nurse maid's elbow." Child A was seen at the emergency room and then had follow up care at Helen DeVos Children's Hospital. She had her arm casted for 2-3 weeks due to concerns there was also a fracture. Child A's Mother was upset the injury occurred, prior to this event, she has had no concerns with her child's care. Child A continues in care with the licensee. On 09/29/2022 I conducted an unannounced on-site inspection to the licensee's home. Ms. Lewis was present with 3 children in care. She explained the incident to me. She said Child A woke up from her nap and she reach out her hands. The licensee had her grab her thumbs to lift her out, "I lifted her about 6 inches and heard a pop." She brought her downstairs and noticed she could not use her arm. She contacted her parent's and Child A was picked up and received medical treatment. Ms. Lewis acknowledged she should not have lifted her up in this manner. I saw the pack-n-play where Child A was sleeping and photographed it. On 10/11/2022 Child A's Mother emailed the physician's report regarding her injury. The diagnosis was nurse maid elbow with no evidence of a fracture.
On 09/15/2022, Jamie Lewis, picked up Child A, female age 20 months, by the hands causing nurse maid elbow and a fracture.
