(1) Food and beverages provided by a center must be of sufficient quantity and nutritional quality to provide for the dietary needs of each child according to the minimum meal 4 requirements of the child and adult care food program (CACFP), as administered by the Michigan department of education, based on 7 CFR part 226, 1-1-18 edition, (2018) of the United States Department of Agriculture, Food and Nutrition Service, CACFP, and is hereby adopted by reference. A copy can be obtained at no cost from CACFP at http://www.fns.usda.gov/cacfp/meals-and-snacks. In addition, a copy is available for inspection and distribution at no cost at the Michigan Department of Licensing and Regulatory Affairs, Bureau of Community and Health Systems, Child Care Division, 611 West Ottawa Street, Lansing, MI 48933.
On 03/15/2023 I interviewed the complainant via telephone. Children are not being served what is posted on the menu. Children are served 2 small portions and are not allowed to have more. For instance, children are served one pancake at breakfast or two tablespoons of oatmeal in a tiny Styrofoam bowl. The children are not served actual fruit. For instance, jelly would be counted as fruit, or the fruit in breakfast bars or the artificially flavored oatmeal fruit. Milk is only served occasionally at breakfast. On 03/23/2023 I completed an on-site inspection at TOTally KIDS. I observed lunch being served. The children were served chicken and dumpling soup which contained vegetables, crackers, peaches, and milk. The children were served the soup in a Styrofoam bowl, that I measured to hold 1/2 cup serving. There was a large bowl of soup in each classroom for the children to request more. I reviewed the menu dated 03/23/2023 and the children were served what was listed on the menu. I interviewed program director Stacey Reynolds. TOTally KIDS participates with the food program. Scoopers are used to measure out the appropriate servings and the children can eat as much as they want. Oatmeal is served with real strawberries mixed into it, jelly is not counted as a fruit and children would be served a fruit on the side. I observed the kitchen. There were bananas on the counter as well as at least 13 large cans of fruit from Gordon Food Service in the cupboard. I observed four gallons of milk in the fridge. I reviewed the March 2023 menus; there were no concerns. I interviewed child care staff member Julia Modrzeiwsaki. She has worked at TOTally KIDS for two years; she is not present at breakfast. Fruit and Milk are served with lunch. Children are always allowed to have more food if they want more. She has never had any complaints from parents regarding food. She has no concerns with the food that is provided to children in care. I interviewed child care staff member Kaylee Travis. She has worked at TOTally KIDS since June 2020. She serves breakfast. If oatmeal is served, frozen fruit is mixed with the oatmeal and served in small bowls. If toast and jelly is served, then fruit would be served on the side. The children are given more if they ask. Children are served fruit, vegetables, and milk with lunch. She has no concerns with the food that is provided to children in care. I interviewed child care staff member Tiffani McDonald. She has worked at TOTally KIDS for four years. This morning breakfast was pancakes, bananas and milk. I confirmed this on the menu. Children are always given more food if they want more. Fruit and milk are always served at breakfast and lunch. She has never had any complaints from parents regarding food. She has no concerns with the food that is provided to children in care. I interviewed child care staff member Keri Croteau. She has worked for TOTally KIDS for three weeks; she is not present at breakfast. Fruit, vegetables and milk are 3 served at lunch daily. Children always get more food if they want; some children have third and fourth helpings. She has no concerns with the food that is provided to children in care. Her three children attended TOTally KIDS before she started working for them; it's a great place. I interviewed child care staff member Christina Jore. She has worked for TOTally KIDS since June 2022. Fruit and milk are served with breakfast and fruit, vegetables and milk are served with lunch. She has never had any complaints from parents regarding food. She has no concerns with the food that is provided to children in care. On 04/19/2023 I interviewed Patricia Yoder from the association for child development food program via telephone. She last observed TOTally KIDS four months ago. She observed lunch and there were no concern with food service. On 04/19/2023 I interviewed Michelle Stickley from the association for child development food program via telephone. R 400.8158 · R 400.8158 Incident, accident, injury, illness, death, fire reporting. (4) A licensee, licensee designee, or program director shall submit a written report to the department of the occurrences outlined in subrules (1), (2), and (3) of this rule, in a format provided by the department, within 72 hours of the verbal report to the department.
On 12/05/2022 I interviewed Child A's Mother via telephone. On 11/15/2022 she picked Child A up from the center around 3:30pm. Child A usually runs up to her when she gets there but on this day she did not. Child A was playing with toys instead and just looked at her. Program director, Ms. Stacy Reynolds and child care staff member, Ms. Joanna Monroe came to Child A's Mother and told her Child A tripped and fell on the carpet. Child A's Mother stated Child A is "clumsy and falls a lot". Child A came over and Child A's Mother noticed Child A had a big abrasion on her left cheek. Child A previously had a small mark on her face from where she fell and hit a wall at home. Per Child A's Mother, the mark Child A had from at home was on the right side of her face. Both sides of Child A's face and the right side of her head were swollen after Child A's Mother picked her up from the center. Child A's Mother called program director, Ms. Reynolds to ask if Child A fell on a toy. Ms. Reynolds reported Child A did not fall on any toys. Child A's Mother asked if there were cameras at the center because she thought it looked like Child A was hit on the side of the head and then fell. Ms. Reynolds reported that there are no cameras at the center. Child A's Mother then took Child A to the doctor on 11/16/2022. Child A was assessed for a concussion, but the doctor did not feel she had a concussion. Child A's Mother was told to keep an eye on Child A and bring her back if necessary. I asked Child A's Mother if she had a copy of the medical report for the 11/16/2022 doctor's visit that she could share with me. Child A's Mother reported she did not have one, but she would get one for me. As of the date 2 of this report Child A's Mother has not sent me a copy of the medical report. Child A's Mother expressed concerns that the explanation of the injury to Child A from the center staff does not make sense to her. When Child A's Mother told program director, Ms. Reynolds Child A would not be returning to care, Ms. Reynolds told her she would not hold her to the two weeks' notice. That made Child A's Mother think there was more to the story. Child A's Mother texted me pictures later on 12/05/2022. The pictures Child A's Mother reported were from 12/16/2022. Child A had a small "scrape type mark" under her left eye and a small red mark under her right eye. The marks are present but not as noticeable as the marks in the pictures Child A's Mother reported were from 11/17/2022. The marks on Child A's face in the pictures Child A's Mother reported were taken on 11/17/2022 were much more noticeable. The marks in the 11/17/2022 pictures were really pink "abrasive type" marks and they were prominent. On 12/05/2022 I completed an onsite inspection. I interviewed program director Ms. Stacey Reynolds. Ms. Reynolds reported on 11/15/2022 Child A arrived at the center with a mark on her face. Child A's Mother told her Child A fell off a couch over the weekend. Ms. Reynold's has text messages from Child A's Mother saying Child A had a mark on her face 11/15/2022 prior to coming to care that day. Later that day during music and movement Child A fell on the carpet. They did not notice a mark at the time. There was redness but no mark. Even when Child A's Mother picked Child A up there was no mark. The next day on 11/16/2022 Child A's Mother sent a text to her to say Child A was going to stay home that day because they "went to camp" the night before and were up late. Child A's Mother also asked what Child A fell on when she fell on 11/15/2022. She said Child A only had a small mark when she dropped her off at the center and now has a big mark on the left with swelling on both sides. Ms. Reynolds reported Child A is always falling. I reviewed the communication log in the 2 3 year old room on 12/05/2022. The 1/2- log contained an entry dated 11/15/2022 which stated, Child A tripped over her own feet and fell on her face. On 06/16/2021 I interviewed the complainant via telephone. Tiffani McDonald has been acting as lead caregiver without the lead caregiver qualification for longer than 90 days. Ms. McDonald works on CEUs while working/rocking babies. On 06/22/2021 I interviewed licensee Stacey Reynolds. Child care staff members do not work on CEUs during child care hours. On 07/20/2021 I interviewed child care staff members Tiffani McDonald, Meghan Baker and Dawn Logan via telephone. No concerns were reported. On 07/21/2021 I reviewed lead caregiver qualifications for child care staff members Tiffani McDonald and Kaylee Travis. Ms. McDonald became a substitute lead caregiver on 06/04/2021. She met the lead caregiver qualification for infant/toddlers on 06/28/2021. Ms. Travis became a substitute lead caregiver on 06/14/2021. As of the date of this report verification was not received that Ms. Travis completed the lead caregiver qualifications. On 07/26/2021 I interviewed child care staff members Chloe Brothers and Kyle Buelterman via telephone. No concerns were reported. On 07/27/2021 I Interviewed Child D (female, 9 years) , E (female, 4 years) and F (female, 4 years)'s mother, Child G (male, 23 months)'s mother and Child H (male, 31 months)'s mother via telephone. No concerns were reported. 4 On 07/282021 I interviewed Child I (male, 32 months) And J (female, 9 months)'s mother and Child K (female, 13 months)'s mother via telephone. No concerns were reported. On 0728/2021 I interviewed child care staff members Scott Mansfield and Kaylee Travis via telephone. No concerns were reported. On 07/30/2021 I interviewed child care staff member Paige Blalock via telephone. No concerns were reported. On 08/02/2021 I interviewed child care staff member Joanna Monroe via telephone. No concerns were reported.

On 06/16/2021 the complainant stated that there are many days that the infant room and 3 years and older room is out of ratio. On 06/22/2021 I reviewed children and child care staff member attendance dated 06/01/2021 and 06/18/2021. The child care center was out of ratio on the following dates: 06/09/2021 There were 14 children and one child care staff member present in 3 years of age and older room from 7:45am — 8:30am; Two child care staff members were required. 06/10/2021 There were 5 children and one child care staff member present in the 12 months — 30 months room from 3:00pm — 4:35pm; Two child care staff members were required. 5 06/11/2021 There were 5-6 children and one child care staff member present in the birth — 12 months room from 7:47am — 8:00am; Two child care staff members were required. 06/14/2021 There were 5 children and one child care staff member present in the birth — 12 months room from 4:00pm — 4:10pm; Two child care staff members were required. There were 5 children and one child care staff member present in the 12 months — 30 months room from 8:50am — 9:00am; Two child care staff members were required. On 06/17/2021 there were 21 children and two child care staff members present in 3 years of age and older room from 12:00pm — 1:15pm; three child care staff members were required. On 06/18/2021 there were 21 children and two child care staff members present in 3 years of age and older room from 12:00pm — 12:15pm; three child care staff members were required. On 07/20/2021 Child care staff members Tiffani McDonald, Meghan Baker and Dawn Logan reported no ratio issues. On 07/26/2021 Child care staff members Chloe Brothers and Kyle Buelterman reported no ratio issues. On 07/27/2021 Child D, E and F's mother, and Child H's mother reported no ratio concerns. Child G's mother had concerns that she has observed up to ten children present in the toddler room with only two child care staff members present. On 07/282021 Child I and J's mother and Child K's mother reported no ratio concerns. On 0728/2021 Child care staff members Scott Mansfield and Kaylee Travis reported no ratio issues. On 07/30/2021 Child care staff member Paige Blalock reported no ratio issues. On 07/30/2021 I interviewed licensee Stacey Reynolds. The child care center is never out of ratio.
