    On 08/22/2022 1 interviewed the complainant via telephone. There is a video of Child K in the infant room on 08/16/2022. Child K is sitting at the table. Child care staff member Ashleigh Vanderleek puts the bottle in Child K's mouth then pulls the bottle out and states that Child K is not drinking it. Child K was left in a swing to sleep for 45 minutes on 08/16/2022. At pick up on 08/16/2022 Child K had been lying on her belly screaming; she was red in the face and covered in snot and tears. Staff is propping bottles in the infant room. On 08/22/2022 I interviewed child care staff member Zoe Johnson via telephone. Since Child K started attending Great Lakes Childcare Center staff have said they do not like Child K and were standoffish towards her. Within the last two weeks things have gotten worse. Staff have been leaving Child K to cry for long periods of time, neglect giving her bottles and neglect changing her. Staff was neglecting her because they don't like Child K. Staff would call Child K's mother to pick her up. Ms. Vanderleek and Ms. Finch took videos and sent them to Child K's mother on 08/16/2022. If Child K is held, she would take a bottle . When she worked in the infant room she figured out Child K's routine and communicated that routine with other staff. She has quit working at Great Lakes Childcare Center because she couldn't take the disrespect towards her and the child care children. She heard Ms. Howard say, " turn that thing away from me, she's so ugly, she looks like a baby bird", in regard to Child K. Bottle propping was common practice in the infant room. She also observed Child Y(male, 5 months) swaddled and sleeping in a swing with a propped bottle. When licensee designee Delynn Pauly is at the child care center, she makes sure things are done correctly. Infants are moved out of swings and bottles are held by staff. A few weeks ago, Child K had fallen asleep in the infant swing and Ms. Finch told her to leave Child K in the swing to sleep. On 08/22/2022 I viewed the two videos sent to Child K's mother on 08/16/2022. The first vides is an 11 second video of Child K in an excer-saucer screaming. No staff members attempt to console her. The second video is a 35 second video of Child K sitting upright at the table. Ms. Vanderleek is attempting to give her a bottle and says" she's not drinking it, she's wearing it". Ms. Vanderleek does not attempt to hold Child K to feed her the bottle. On 08/22/2022 I reviewed a group staff text message at Great Lakes Childcare Center. Ms. Howard sent the group a "butt hurt" report and writes "some days I think 4 we need these". Ms. Finch writes "for parents to". Ms. Howard writes "like Child K's mom today lol". Ms. Finch laughed at Ms. Howard's comment. On 08/23/2022 I completed an on-site inspection at Great Lakes Childcare Center. I interviewed licensee designee and program director Mariah Finch. They do not prop bottles or leave children in swings. Child K was an inconsolable baby. Child K's mother picked her up on 08/16/2022 and pulled her out of care because she knew it wasn't a good fit. Child K's last day was Tuesday 08/16/2022; she was present from 9:03 am-11:43 am. They asked Child K's mom to pick up Child K on 08/16/2022 because staff could not console her. On 08/17/2022 Child K's father asked to review the video footage from the infant room that day. Ms. Johnson quit on 08/16/2022. She admits the staff group texts were in bad taste. Ms. Howard did say in a staff meeting regarding Child K "she is ugly". She told Ms. Howard that she can't talk like that. I reviewed the video footage from 08/17/2022 with Ms. Finch. At 9:23am Ms. Vanderleek puts Child K in an infant swing. At 9:33am Ms. Vanderleek turns the swing away from the classroom and the camera. At 9:45am Child K appears to stop moving around and falls asleep. At 10:12am Child K appears to be awake and moving around. At 10:21a m child care staff member Alicia Ballor takes Child K out of the swing and changes

    Licensee designees Mariah Finch and Delynn Pauly failed to submit a written report to the department regarding incidents that occurred at the child care center. On 07/28/2022 there were 14 children under 30 months being supervised by two child care staff members in the unfenced outdoor play area. There is sufficient evidence that Child V was able to run 89 feet and reach the child care center parking lot before a staff member caught Child V. In January 2022 child care staff member Aslinn Westbrook fell asleep while caring for two infants.

    Licensee designees Mariah Finch and Delynn Pauly failed to ensure that swings were not used for sleeping. On 08/16/2022 video shows that Child K was left in a swing to sleep for 27 minutes while being cared for by Ms. Vanderleek and Ms. Ballor. Child care staff members Zoe Johnson, Ashleigh Vanderleek, Alicia Ballor and Amanda Hilton all reported observing Child K sleeping in a swing on one or more occasions. Ms. Johnson reported that a few weeks ago Child K had fallen asleep in the infant swing and Ms. Finch told her to leave Child K in the swing to sleep. Ms. Gies observed infants sleeping in swings. Four child care parents reported observing infants sleeping in swinas at the child care center.

On 08/16/2022 Ms. Vanderleek is observed in a video attempting to feed Child K sitting up at the table and says" she's not drinking it, she's wearing it". Ms. Vanderleek picks Child K up and lays down next to her and feeds her one ounce of a bottle while Child K is lying on a boppy pillow. Ms. Vanderleek attempts to get Child K to hold her bottle. Child K will not hold the bottle. Ms. Vanderleek sets the bottle down and gets up, leavina Child K on the floor.

    On 01/25/2022 I completed an on-site inspection at Great Lakes Childcare Center. I interviewed licensee designee and program director Mariah Finch. Child A (female, 2 years 8 months) is in the 2 - 3-year-old classroom. This 1/2 classroom has up to six children and one or two staff members. On 01/18/2022 child care staff member Taylor Bromund was in that classroom. Ms. Bromund contacted her at 12:55 pm to let her know that Child A was covered in feces. They informed Child A's parents, cleaned Child A up and Child A's mother picked her up. Child A's mother was embarrassed at pick up and stated Child A had never done this before. The children go down for nap time at 11:50am. Child A had a clean diaper when she went down for nap. Child A does not usually sleep during nap time. I reviewed the classroom video from 01/18/2022. There were six children ages 2 1/2 - 3-years-old present, and I observed two child care staff members in the classroom until 12:20 pm. All the children were resting on cots. Child A was moving around 2 and fidgeting on her cot throughout the entire video. At 12:45 pm Child A appears to put her hand in her diaper. At about 12:53 pm Ms. Bromund notices that Child A is covered in feces and makes a phone call to the office for help. I interviewed child care staff member Sadie Chabot. She was helping out in the 2 1/2 - 3-year-old classroom on 01/18/2022. She was not in the classroom when the incident occurred with Child A. Nap time is difficult for Child A and she does not sleep. Child A does like to play in her diaper at nap time, but never to this extent. Child A will normally tell a child care staff member if she needs something. She has no concerns with the care and supervision children receive at Great Lakes Childcare Center. I interviewed child care staff member Taylor Bromund. She has been the lead in the 2 - 3-year-old classroom since January and has had no issues with Child A 1/2 playing in her diaper. She is always fidgety and moving at nap time; she never sleeps. They have tried giving Child A different quiet activities at nap time but nothing seems to work. On 01/18/2022 she noticed Child A fidgeting. She smelled feces, then the smell became stronger. She saw brown on Child A and shined her flashlight on Child A to get a better look. She immediately called Ms. Finch for help. She has no concerns with the care and supervision children receive at Great Lakes Childcare Center. I interviewed child care staff member Paige Thompson. She works in the 2 - 3- 1/2 year-old classroom but was off on 01/18/2022. There have been no issues with Child A playing in her diaper in the past. She moves around a lot at nap time. She has no concerns with the care and supervision children receive at Great Lakes Childcare Center; children are well taken care of. I observe the 2 - 3-year-old classroom at nap time. I did not observe any blind 1/2 spots in the classroom. There was sufficient light in the classroom to observe children. On 02/23/2022 I interviewed Child B (female, 2 years 8 months) and Child C (male, 9 months)'s mother via telephone. Child B and C have attended Great Lakes Childcare Center since December 2021. She has no concerns with the care and supervision Child B and C receive at Great Lakes Childcare Center. On 03/02/2022 I interviewed Child D (male, 2 years 9 months)'s mother via telephone. Child D has attended Great Lakes Childcare Center since they opened in September 2021. She has no concerns with the supervision Child D receives at Great Lakes Childcare Center. In 03/02/2022 I interviewed Child A's mother via telephone. Child A had COVID in November and has had stomach issues and diarrhea since. It is not normal for Child A to play in her feces. Child A does not sleep during nap time. On 01/18/2022 Ms. Finch texted her a picture of Child A covered in feces and let her know what 3 happened. The staff cleaned Child A up and changed her clothes before she picked up Child A. She has no concerns with the care an
