    Based on interviews, video review, and documentation, program staff were not positioned to adequately supervise the area where the incident 2 LANDMARK BUILDING • 105 W. ALLEGAN STREET • LANSING, MICHIGAN 48933 Michigan.gov/MiLEAP occurred. Although the child received immediate attention after the injury and the initial notification was sent to the parent, the lack of follow-up response to the parent’s communication on the application prevented the parent from asking additional questions and obtaining timely information regarding the child’s condition. This breakdown in communication reflects a failure to fully monitor and respond to the child’s needs and the parent’s ability to make informed decisions regarding care, which supports that appropriate care and supervision was not maintained.
