On 04/27/2023, the center submitted an incident report detailing the allegation. On 04/29/2023, 1 completed an unannounced on-site inspection and interviewed CCSM 1 who reported that the name that Child A said was is shared by two children at the center. One, Child B, is in the classroom with Child A and the second, Child D, and the other child could have contact with both of them in the bathroom and playground. Child A, Child B and Child C all sit at the same table for snacks and meals. CCSM 1 reported that she thought the allegation was bizarre and she was not sure where it is coming from. She said that there had been some potty talk in the classroom and recalled that Child B called another CCSM "toilet head." She said that Child B sometimes makes up stories and sometimes she cannot tell that it is not true. CCSM 1 does not think that the allegations are true. CCSM 1 described Child B as "very active, bright" who does not usually interact with Child A. Child A and Child D play together. She said that Child A does not know the names of all the children. A couple months ago, Child A reported that someone her in the Ro but the name of the person that she said _ her was not someone attends the center. CCSM 1 said that children are never unsupervised at the center and there is always a child care staff member with them. I interviewed CCSM 2 who reported that children are never unsupervised and there are a lot of child care staff members there. The children are not alone in the bathroom. CCSM 2 has never seen any children with their_,. She has no knowledge of the allegation. I interviewed CCSM 3 who reported that she has heard boys talking about butts, but she has never heard anyone talk about inappropriate touching. She has not seen children with their pants down. There has not been any inappropriate behavior in the bathroom. Children are never unsupervised in the classroom, outside or in the bathroom. On 05/04/2023, 1 spoke with Child A's Mother and Child A's Father viottele hone. Child A's Father said that Child A asked her mom if itwas ok toll Child A's Mother asked Child A where she of that i ea r i II said rom school and then said that boys have girls Child A's Mother asked her if it happened to her. Child answered no and said that Child D keeps her safe. Two months prior, Child A said something about and then said Child E Child A's Father told CCSM 1 about what i said but did not tell her that i E was the one who did it. Child A's Father and Mother said that Child A has never talked about M in their home, so they are not sure where that came from. Child A's Father Child A seems like she is not comfortable at the center. Child A did not say 3 which child was being Child A's Father said that when he is in the center, he sees child care staff members near and usually in the bathroom when children are in it. He has picked Child A up during nap time and the children were being supervised at that time. On 05/17/2023, 1 interviewed program director Daphnea Sutherland who reported that Child A's Father shared with her what Child A had said. He told her that Child A asked if it was ok if and the name of the child who ChildA's Father sai a Child A said that it has not happened o her because Child E her. Daphnea Sutherland said that she _ spoke with her staff and none of them have seen anything inappropriate. There have been no conversations among children about inappropriate contact. I asked Daphnea Sutherland about what Child A said about Child E. Daphnea Sutherland reported that Child A's Father did not talk to her about it. She said that Child E does not have and to her knowledge none of the children enrolled at the center do"et r. On 07/05/2023, 1 spoke with Child B's Mother who reported that she has no concerns about the allegations, or the supervision and care provided to Child B. Child B has not had any inappropriate behaviors or contact. She has had nothing but a good experience so far with the center. I spoke with Child C's

On 02/04/2021 I received a phone call from Program Director Daphnea Sutherland, who stated that Child A had been left unattended on the playground that same day, for around 10-15 minutes. She stated that the child was brought inside and appeared to be ok. That same day Ms. Sutherland emailed me a copy of the incident report and a written statement by the lead child care staff member in the Preschool 1 classroom, Abigail Galavis. The statement indicated that the class had been out on the playground that morning and had started transitioning back inside around 10:28am. Ms. Galavis came inside with several children at that time, and child care staff member Samantha Behen had come in shortly thereafter with what they thought was the remainder of the children. They got the children's snow gear off and transitioned to bathroom time. When they got to the bathroom and were mid-way through bathroom time, they noticed that Child A was not with them. Ms. Galaviz went to the classroom and saw that he was not there either, and she looked outside and found him there. She stated that she looked at her watch at that time and it was 10:45am. On 02/11/2021 I completed an unannounced on-site inspection at the center. I interviewed child care staff member Abigail Galavis. She stated she has been working in the Preschool 1 classroom since September 2020 as the lead, and Ms. Behen has been working in the room with her. On 02/04/2021 they were outside for morning recess and had "10 or 11 kids." She said she wasn't sure the total number and said, "I've been bad at that lately. It was a higher number of kids that day, but there's no excuse for it. I should have known the exact number." Ms. Galavis explained the class was outside for around 15 minutes and she came in at 10:28am with about half the class. The other half was finishing up sledding with Ms. Behen and came inside as they finished. "Sammi came in shortly after with the rest of the kids." I asked Ms. Galaviz if they counted the children as they came in and she said they did not. "Normally we do the name to face sheet when we leave the building and when we enter. I failed to do it both times." They helped the children take off their snow stuff and then had them put their shoes on, line up and walk down to the bathroom which is at the other end of the building. About halfway through their bathroom routine, Ms. Galaviz noticed that Child A was not in line with the other children. She said, "Where's [Child A]?" and Ms. Behen replied, "I don't know." She walked back to the room and he was not there. "I found him right outside the door and I brought him in. I tried to keep him calm and not act like it was a big deal. It was 10:45am when I got him, and it was 10:50 when I notified Daphnea." Ms. Galaviz said she spoke with Child A's Mother and she didn't seem to have concerns for safety. She said, "it happens." "Sammi was the last one out, but it was my fault, because I'm the lead. It was my responsibility." Ms. Galaviz said she was very distraught at the incident, as nothing like this has happened in her 7 year experience 3 in classrooms. That same day she asked Ms. Sutherland if they could implement change to their supervision policies to make sure it didn't happen again, and Ms. Sutherland told her she could write up her ideas for them to look over. "I implemented a change. We are now teaching the staff to use active supervision. Always know your count, know the kids in your care, their behaviors and personalities. If you have a kid that likes to hide or run, you should know that and be on top of it. We are also now verbalizing our count with our teaching partner. Both teachers say the count back to one another anytime you transition and randomly throughout the day. We also put boards up in each room which has the current number of children on it, which is updated all day. Anyone entering a classroom is expected to count and confirm the number of kids on the board." Ms. Galaviz stated that she too
