CCSM 1 and CCSM 2 provided appropriate care and supervision in the classroom when Child A injured their mouth. CCSM 1 and CCSM 2 observed the incident that caused Child A's injury on 7/30/2025 and CCSM 2 immediately provided first aid to Child A with the help of CCSM 3.

On June 15, 2023, the licensee, Kristeena Ramey, sent an incident report regarding these allegations via email. On June 21, 2023, I initiated this investigation via telephone with Child A's Mother who reported that Child A was injured at this child care facility on June 14, 2023. She stated that she was called by CCSM 2, as she was arriving at the child care center for pick up. Child A was taken to the dentist as the incident resulted in injury to his tooth. The front tooth was loose from the incident. Although Child A's Mother stated that she has no concerns with the care and supervision provided, she said that the staff members are not able to tell her how Child A's tooth was injured. Child A's Mother said, while at the center "they" took Child A, and two other children that he was playing with on the day of the incident, outside to the area in an attempt to have them point out what happened. Child A tried to tell her what happened, but it was unclear to her if he was saying, "ledge", "bridge", or "edge". Child A is still enrolled in care. On June 22, 2023, during an unannounced on-site inspection, neither program directors, Kristeen Ramsey nor Andrea Rushing were present. Childcare Staff Member (CCSM) 1 was present. CCSM 1 stated that she is aware of the incident, however, Child A is not in her class and therefore, she was not outside during the incident. CCSM 1 called Ms. Ramsey and I interviewed her via telephone on site. Ms. Ramsey reported that she is aware of the incident but was not present when the accident occurred. Ms. Ramsey could not explain how Child A hurt his tooth. She stated that the injury was minor, Child A was seen by a dentist, and he is "fine". She said that Child A's Mother does not have any concerns and she does not know why a complaint was made. On June 22, 2023, CCSM 2 was interviewed via telephone. She reported that she was not outside when Child A was injured. She and CCSM 3 were inside completing Father's Day gifts. She said that CCSM 4 was outside alone with their class. CCSM 2 stated that when Child A was injured, she brought him inside to administer First Aid. She said she tried to gather information from the children but could not get a clear story. On June 22, 2023, CCSM 3 was interviewed via telephone. She said that she was inside the building assisting CCSM 2 with Father's Day gifts. She did not see the incident. On June 23, 2023, CCSM 4 was interviewed via telephone. CCSM 4 said that she was outside when Child A was injured. She said that she was helping some children who were playing at the sand table when Child A ran to her saying "I got owwie!" His mouth was bleeding. She did not see how he was injured. She called for assistance. Child A was taken inside by CCSM 2 to received first aid. She remained outside with the class. CCSM 4 stated that she was outside alone with about 10 3- year-old children. 4 On June 29, 2023, 1 received the attendance record from the date in question. The incident report indicated that the incident occurred on June 14, 2023, at 5:15pm. The attendance record shows Child A being signed out on June 14, 2023, at 5:10pm. The record shows that there were 7 children present between 5:00pm and 6:00pm. On August 8, 2023, Child B's Mother and Child C's Mother was interviewed during separate telephone calls. They each reported having no concerns with the care and supervision received at this child care center.

The center self-reported to the department that there were allegations that Alexis Walker smacked Child and Child B in the mouth for spitting. On 07/14/2022, I completed a telephone interview with CCSM, Brittany Fell. Ms. Fell began working at the center on 5/31/2022. On Friday 07/08/2022, Ms. Fell was working in the toddler room with Alexis Walker and CCSM 1. CCSM 1 went to the bathroom and Ms. Fell and Ms. Walker were in the room with the children getting ready for lunch. Child A and Child B were at a table spitting. Both Ms. Fell and Ms. Walker were telling the children to stop spitting however, they did not. Ms. Fell stated that Ms. Walker went over to Child A and Child B and smacked them in the mouth with the front of her hand. Ms. Walker hit Child A then hit Child B. Child A continued to spit so Ms. Walker hit Child A again. Child A then began to cry. Ms. Walker looked at Ms. Fell and shrugged her shoulders and said, "What, they are spitting." CCSM 1 returned from the bathroom and asked if Child A was ok. Ms. Walker told her Child A was fine. Ms. Fell did not say anything. The staff members then served lunch. None of the staff members comforted Child A. Ms. Fell told CCSM 1 after Ms. Walker left. Ms. Fell did not want to confront Ms. Walker because she was worried that it would lead to a dispute or cause a further issue. CCSM 1 told Ms. Fell to email the directors after her shift ended. CCSM 1 went to ask another staff member how they should handle the situation. CCSM 2 told both CCSM 1 and Ms. Fell to immediately call the directors. Both program directors were not on site and there was not another lead staff member present with them at the time. Ms. Fell contacted licensee and program director Andreea Rushing and told her about the incident. Ms. Rushing told her that the next time something like that occurs she should contact them immediately so that they could stop the staff member from working. Ms. Fell's last day at the center was 7/11/2022, the directors told her that she would be off during the investigation. Ms. Fell stated that she did not know the center's discipline policy or prohibited means of discipline. On 07/18/2022, I completed an onsite inspection at Little Jungle Learning Center II. I interviewed licensee and program director Andreea Rushing, CCSM 1, and CCSM 2. Andreea Rushing, stated that on 07/08/2022 both she and the other program director were off that day. She received a call around 3:34 p.m. from Ms. Fell informing her of the same information that Ms. Fell stated above. She informed Ms. Fell that she should have called her immediately. The incident occurred around 12:30 p.m. and Ms. Walker left at 2:00 p.m. Ms. Rushing feels that there was friction between Ms. Fell and Ms. Walker. Ms. Walker was a hard worker always willing to pick up extra shifts. Ms. Rushing does not feel that other staff members melded well with Ms. Walker. Ms. Walker was, "very on the ball and with it maybe rubbed staff wrong because she was always so on." Ms. Walker has not been onsite since 7/10/2022 and Ms. Fell since 7/12/2022. Ms. Rushing does not think that Ms. 3 Walker would hit a child. Child B continues to attend the center. Child B's Mother told Ms. Rushing that she would have assumed that Ms. Fell would have been the one to hit a child, not Ms. Walker. Child A continued to attend for an additional week then her family disenrolled her. Ms. Rushing provided me the center's website. I viewed the centers discipline policy on their website. CCSM 1 has been working at the center since 9/2021. CCSM 1 stated that she knew the centers discipline policy but could not exactly remember what it was. CCSM 1 stated that she learned of the discipline policy when first hired. CCSM 1 does not really know the prohibited means of discipline. CCSM 1 has received training on proper discipline. If a child is spitting staff are to tell the child to stop spitting and explain that it is nasty and tell them "gentle". The children know this t

I was provided a copy of the food schedule. According to the daily schedule, children are fed breakfast at 7:30 am. upon arrival. A snack is provided between 9:00 a.ma-10:00 a.m. Lunch is served from 12:00 p.m.-1:00 pm. A second snack is to be provided from 3:00 p.m.-3:30 p.m., and the children are allowed another snack when they leave to go home. 4 According to the program directors, Ms. Rushing and Ms. Ramey, they provide more than the required amount of food to the children. They denied the allegations that children are underfed or eat out of the trash. They had not been informed or confronted with any concerns of the lack of food being served to any of the children. Child care staff members, Ms. Mchenry, Ms. Anthony, Ms Brunson and Ms. Meagan Jabberbrow was unaware of any concerns that the children were not being fed enough or any allegations of any children eating out of the trash. CCSM, Ms. Brunson stated that there was one child that use to attend the center that had attempted to grab some food out of the trash, but the child was always stopped. There was always a lid on the trashcan. This incident was never reported to the directors as it was an isolated incident. The child had not complained about being hungry. The parents of Child A, Child B, Child C had no concerns about the amount of food the children were being fed while at the center. There were no children identified or named in the allegation that had allegedly eaten out of the trash can.

Child C sustained an ankle injury while at the center and had to receive medical treatment. Child C's Mother reported she communicated thru tadpole that Child C would not return to the center due to injury. Child C did not return to the center for several weeks, and when she returned, she had a boot on the same ankle that was injured at the center. Child C's Father said he told a CCSM that he was planning to take Child C to the doctor while picking him up following the accident. No verbal report was made to the department within the required 24-hour time frame.
