On 0411412021, Child A year-old, was hit by a caregiver at Kidz World Learning Center. 0 C hild A as an abrasion on ■ left cheek and two or three nail marks behind left ear. E On 04/1612021, and I completed an unannounced onsite inspection at Kidz World Learning Center. We spoke to Heather Selonke, assistant director, and child care staff member regarding the allegation. We observed the entire chid care. In the Infant room, there were eight infants with two child care staff members. In the Toddler room, there were eight children with two child care staff members. In the Young Preschool room, there were eight children with one child care staff member. In the Preschool room, there were 16 children with two child care staff members. A copy of attendance sheet from 04/14/2021, incident report, Brightwheel documentation, and Child Information card for Child A, B and C was obtained. According to Ms. Selonke, Child A is enrolled at the child care center in the '~ room and is the child care staff member for that room. There are usually eight to 10 children, ages , in the room with one child care staff member. According to Ms. Selonke, Child as been attending the center for a while. Child A has temper tantrums when. is unable to get way. When was in the room, the temper tantrums were daily. Ms. elonke would assist the child care staff members and removed Child A from the class so that could calm down. temper tantrums have gotten better and occurs less frequen y. On 04/14/2021, s. Selonke was not present at the child care center when the incident occurred. However, she received a message that was sent from Child A's Mother through the Bri htwheel application. The message was a response from a message that sent around 10:45 a.m. had sent Child A's Mother a picture of Child s cheek and of her nails. Child A's Mother responded around 11:00 a.m. Child A's Mother stated that she was in but that she would pick Child A and take to Child A s other called Ms. Selonke but she was at an appointmen and was unable to answer the phone. Child A was picked up from the center at 3:51 p.m. Around 5:13 p.m., Child A's Mother facetime Ms. Selonke's cell phone. Child A's 3 Mother wanted Ms. Selonke to see the scratch on Child A's face. They were at Child's Mother stated, "Look at this. The is saying this is not a scratch." Ms. Selonke told Child A's Motherthat t e scratch looked worse than the picture that was on the Brightwhe4aat lication. Child A's Mother called later that evening and inform Ms. Selonke was coming to her home regarding the incident. Ms. Selonke spoke to the following morning and had her complete an incident report. Acco o , Child A and Child C -year-old, }were fighting and was trying to separate the two children. After the incident she seen the scratch on Child A's face, and she contacted Child A's Mother. She was unsure how Child A was scratched. Pr was the only caregiver in the room at the time of the incident. ■ been employed at Kidz World for the ast years. There is no isciplinary action regarding T ere is no video footage of the incident. On 04116/2021, was interviewed regarding the allegations. She stated on 04/14/2021, she had t children in the room. They were being very ruff with the toys, and she told them if they did not stop, they would have to put them away. They continue to play ruffly and she told them to put the toys away. Child A got upset and the other two children begin putting up the toys. Child A threw an empty plastic container and it hit Child C in the back of the head. On 8/12/2022, licensing consultant Brandon Robinson and I conducted a joint inspection at the center. We interviewed Whitney Anderson, acting program director. Ms. Anderson stated on 7/27/2022, Child A was having a tantrum, throwing chairs and hitting at other children. She was brought to the office by CCSM Ms. Susan. She stated Ms. Susan was guiding Child A with her hand on her back. Child A's Mother was contacted and came to the center. Child A told her mother Ms. Susan hit her. Child A bit Ms. Susan on the leg during this encounter. Child A 2 continued to attend the school. On 8/10/2022, Child A had another outburst in the classroom. Child A was carried into the office by Ms. Heleena. Ms. Heleena had Child A in front of her facing forward. She had her arms wrapped around Child A. We interviewed CCSM, Ms. Susan Abdul Sater. Ms. Susan stated she loves Child A. She has attended the center a long time along with her brother. She is a good child until she has a tantrum when things do not go her way. Child A will scream, throw things, and hit at other children. Ms. Susan admitted Child A bit her during the tantrum on 7/27/2022. She said she said "Ouch" and told Child A she needed to go to the office. On 8/10/2022, Child A had a tantrum. She was throwing things, screaming, and hitting at the CCSMs and children. She was carried to the office by Ms. Heleena. Parts of her tantrum was captured on video. Ms Susan stated they have talked to Child A's Mother about her behavior, but Child A's Mother laughs it off and says she does not act that way at home. We reviewed the video of 8/10/2022. Child A was in the office screaming and having a tantrum as described. On 8/26/2022, I returned to the center and interviewed Ms. Heleena Sundling. She stated on 8/10/2022, they were supposed to have water day, but it was cancelled. Child A wanted water day and had a tantrum. She was flipping chairs, knocking toys off the shelf, and hitting at Ms. Susan. Ms. Helena scooped Child A up and carried her to the office. She stated she had Child A was facing away from her and her arms around Child A's waist. Child A was still scratching and punching at Ms. Heleena. Ms Helena told Child A if she calmed down, she could return to class. Ms. Heleena stated when Child A's Mother is told about Child A's behaviors, she tends to blame everyone but Child A because they provoke Child A into these behaviors. On 08/16/2022, Brandon Robinson interviewed Child A's Mother regarding a complaint received on Kids World Learning Center. Child A's Mother said she took her daughter to school on 07/27/2022 and got a message from the center that her daughter bit Ms. Susan. She went to the center after receiving the message because Child A does not act out "like that". When she got to the center Child A told her that another child hit her. Child A did not know the name of the child that hit her but kept saying "he hit me". On 08/10/2022, Child A's Mother got a call that Ms. Heleena hit Child A, therefore she returned to the center. Child A's mother stated, Ms. Whitney heard Child A tell her Ms. Heleena hit her on the back, so she bit Ms. Heleena. Ms. Whitney told Child A's Mother Ms. Heleena only grabbed Child A and brought her in the office afterwards. Child A's Mother never said Child A was dragged, squeezed or handled forcefully by staff. She stated her concern is that her child was touched by staff, and that touching children is a direct violation of policy. Child A said "She grabbed me and hurt me" referring to Ms. Heleena. Child A's Mother was told that I observed a video recording of Child A acting out. She responded by saying Child A never acts out and someone had to do something to her. She said Child A is very sensitive and 3 she does lash out and scream when she is upset but someone must do something to her first. On 08/15/2022, Brandon Robinson interviewed Child B's Mother regarding a complaint received on Kids World Learning Center. Child B's Mother said her son attend

If a child has a health issue or special need that requires the child use a swaddle attachment or swaddle wrap after the child is 2 months of age, documentation from the health provider is required. On 8/20/2021, 1 completed an unannounced inspection at the center. I observed the infant room. There were three children present, two were in in their cribs. Neither infant was swaddled. I spoke with assistant program director, Helena Sundling, and child care staff members in the infant room, Christine Hernandez and Zahraa AlBugalal. All three women denied swaddling any infants in the room. They are aware of licensing rules and the rules are posted on the wall in the infant room. They do not use blankets. Ms. Sundling recalls getting a recent phone inquiry about sleep sacks. She stated she told the caller that infants can use sleep sacks and thought maybe there was a confusion between swaddling and sleep sacks.
