    R 400.1911 · R 400.1911 Care; supervision; children. (1) A licensee shall ensure appropriate care and supervision of children at all times.

    On 4/27/2023, I interviewed Child A's Mother who stated that on numerous occasions she's witnessed a few of the after-school children outside playing in the backyard unattended. Child A's Mother said she is not sure the ages of the children she witnessed playing alone outside but believes the children may be around nine years old. On 4/27/2023, I interviewed licensee, Ms. Kimberly Taylor and Child Care Staff Member 1 (CCSM 1) at her childcare group home. Ms. Kimberly Taylor's staff consists of herself and CCSM 1. I went over the allegations with Ms. Taylor. Ms. Kimberly Taylor stated she said she never leaves children outside unattended without adult supervision. Ms. Kimberly Taylor explained that she has herself along with CCSM 1 and she always follows the required 1:6 child care staff member to child ratio during the day. On 4/27/2023, I interviewed CCSM 1. CCSM 1 stated that the child care children along with the school-aged children are never left unattended. CCSM 1 stated that herself along with Ms. Kimberly Taylor are always with the children and she suspects Child A's Mother is very upset because of payment owed to Ms. Kimberly Taylor. CCSM 1 stated that during the short time Child A was enrolled she did not have many encounters with Child A's Mother, Ms. Kimberly Taylor communicated with Child's A Mother most of the times. On 6/23/23, I interviewed Child C's Mother by phone. Child C's Mother stated that she's never observed the children being unsupervised while in care. Child C's Mother stated that she's never had any issues with Ms. Kimberly Taylor and has been very happy since enrolling two years ago. On 6/23/23, I interviewed Child D's Mother by phone. Child D's Mother has never observed children being left unattended and has been very happy with Ms. Kimberly Taylor's day care. Child D's Mother also added that her children are excited to go to daycare every day, they've never complained about going to see Ms. Kimberly Taylor. 3

    On 4/27/23, I interviewed Child A's Mother who said that Ms. Kimberly Taylor admitted to her that Child B likes to hold Child A and that Child B is very responsible. Child A's Mother stated that Ms. Kimberly Taylor told her that she's always on the floor with Child B and Child A. Child A's Mother has never seen Child B physically holding Child A. Child B is the only school-age child who is very fond of Child A. On 4/27/23, I interviewed Ms. Kimberly Taylor in regard to her allowing school-age children to hold infant child care children. Ms. Kimberly Taylor explained that Child B (female, age 10 years old), loves to play and talk with Child A (female, age 2- months) when she's in attendance. Ms. Kimberly Taylor said Child B gets on the floor to play with Child A with another adult but Child B does not hold Child A. Ms. Kimberly Taylor said that she jokingly told Child A's Mother that Child B skips school just to come and play with Child A. Ms. Kimberly Taylor said the comment was a "complete joke," in fact Child A's Mother thought it was "cute" and laughed it off. On 4/27/23, I interviewed CCSM 1 who also stated that she does not allow any of the school-aged children hold any of the child care children. CCSM 1 stated that Child B loves to play with Child A, but she never holds Child A.

    R 400.1916 · 1 R 400.1916 Bedding and sleeping equipment. (6) Infants, birth to 12 months of age, shall rest or sleep alone in an approved crib, play yard, or porta-crib. This equipment must meet all of the following requirements: (a) Cribs, porta-cribs and play yards must comply with the product safety standards issued by the Consumer Product Safety Commission, 16 CFR 1219 (2019), 16 CFR 1220 (2019), and 16 CFR 1221 (2019), which are available at http://www.cpsc.gov. These standards are also available for inspection and distribution at the Michigan Department of Licensing and Regulatory Affairs, Bureau of Community and Health Systems, Child Care Licensing Division, 611 Ottawa, Lansing, MI 48933-1070.

    On 4/27/23, I spoke with Child A's Mother, she stated that Child A did not have a bassinet to sleep in. When she toured the child care there was not a place for Child A to sleep but she was told by Ms. Kimberly Taylor that she will purchase a porta crib for Child A before the first day of attendance. Child A's Mother sent me a video via text message of the child care to show proof that Ms. Kimberly Taylor did not have a place for Child A to sleep. During my onsite inspection on 4/27/23, Ms. Kimberly Taylor offered me a tour of her child care and showed me where Child A sleeps during nap time. Ms. Kimberly Taylor had an appropriate porta crib for Child A to sleep in during her nap times. Ms. Kimberly Taylor denied Child A not having a crib to sleep in nor does she hold Child A during her naps throughout the day. Ms. Kimberly Taylor stated that because Child A is so young, she brings the porta crib into the main area where Child A is visible to her and CCSM 1. Ms. Kimberly Taylor also stated that Child A's Mother came to her home on a Saturday afternoon to tour her child care and was shown where Child A would sleep, at that time Ms. Taylor did not have a crib for Child A, but assured Child A's Mother that she would acquire one before Child A attend her child care home. Ms. Kimberly Taylor felt like Child A's Mother was very pleased with her child care area. During my onsite visit on 4/27/23, I interviewed with CCSM 1 regarding where Child A sleeps during the day. CCSM 1 stated that Child A sleeps in one of the cribs in the back room, but Child A is moved to the front because of her young age and because she takes numerous naps throughout the day. During my on-site inspection on 4/27/23 of Ms. Kimberly Taylor's child care, I observed a room in the back of the child care area where there were with two porta cribs that were not shown in the video that was sent to me via text by Child A's Mother. Ms. Kimberly Taylor assured me this is the space where Child A sleeps and has been since time of enrollment. 5

    On 4/27/23, via phone Child A's Mother stated that she took a pack of 45 diapers for Child A and within eight days Child A only used 23 of those diapers. Child A's Mother believes that Ms. Kimberly Taylor has not been changing Child A as much during the day. Child A's Mother sent me a video via text, but it only showed CCSM ,1 handing Child A's Mother a stack of her remaining diapers. Child A's Mother responds in the video by saying "that is a lot of diapers, I brought in 45 diapers how could she not go through these in three weeks, she didn't change my kid at all." Child A's Mother only spoke to CCSM 1 about this, Child A did not return to child care. On 4/27/23, I interviewed Ms. Kimberly Taylor. She stated that against her better judgement she accepted Child A who was only 2 months old at the time of enrollment. Ms. Kimberly Taylor stated that she typically doesn't enroll infants at such a young age. Child A's Mother asked Ms. Kimberly Taylor to change Child A's n diaper every 20 minutes, but that there were times when Child A's diaper was not wet. Ms. Kimberly Taylor ensured me that she notifies Child A's Mother verbally at the end of each day of Child A's feedings and bowel movements. Ms. Kimberly Taylor also explained that Child A is very young, so she tends to sleep a lot during the day, and she was not a big eater so there weren't many bowel movements. On 4/27/23, I also spoke with CCSM 1 during my on-site inspection. CCSM 1 stated that she was approached by Child A's Mother and an unknown woman at pick up one afternoon. CCSM 1 stated that Child A's Mother was very upset and was recording their conversation. Child A's Mother asked for Child A's diapers. CCSM 1 gave Child A's Mother the remainder of Child A's diapers, which is when Child A's Mother became upset because she believed that Child A should not have had so many diapers left over. CCSM 1 has not had too many interactions with Child A's Mother up until the encounter when she was asked to get the remaining diapers. On 6/23/23, I interviewed Child D's Mother who enrolled her child (female, age 3 months) four years ago. Child D's Mother stated that Ms. Kimberly Taylor took great care of her daughter as an infant and has never had any problems regarding meals or diapering. Ms. Kimberly Taylor is now providing care for her son (male, age 1 1/2 years old).

(1) A licensee shall ensure that all of the following requirements are met: (b) Children Shall be offered food at intervals as individually appropriate, but not to exceed more than 4 hours unless the child is asleep.

    On 4/27/23, Child A's Mother says she believes that Child A was not given enough formula during her time with Ms. Kimberly Taylor. Child A's Mother stated when Child A first started child care, she took 12 oz can of formula and over the course of 7 eight days Child A did not finish the entire can of formula. Child A's Mother believes more of the formula should have been consumed over the eight-day period. On 4/27/23, Ms. Kimberly Taylor stated that she feeds Child A on demand each day. Ms. Kimberly Taylor stated that Child A was not a big eater, and she slept a lot during the day, so she tried to feed Child A each time she was awake. Ms. Kimberly Taylor stated she believes all complaints are from Child A's Mother. Child A only attended her child care for seven days. Ms. Kimberly Taylor explained that she asked Child A's Mother about her payment towards child care and Child A's Mother stated that she should be receiving a payment from the Michigan Department of Health and Human Services for child care, but Ms. Kimberly Taylor had not received any funds for her services. Ms. Kimberly Taylor said the day after inquiring about payment, Child A's Mother came in along with a friend very upset accusing Ms. Kimberly Taylor of not changing Child A when her diaper was soiled and not feeding Child A enough formula during the day. Child A's Mother attempted to record the conversation but was asked not to by Ms. Kimberly Taylor.

    During my interview on 4/27/23 with Child A's Mother, she informed me that in her contract with Ms. Kimberly Taylor it states that her dog will not be in contact with the child care children. Child A's Mother attempted to send me a video via text regarding a conversation between herself and Ms. Kimberly Taylor regarding the dog but the conversation was cut off so I couldn't determine if Ms. Kimberly Taylor admitted to the dog being around the child care children, The video only shows Ms. Kimberly Taylor saying "oh no I have to pick him up and set him outside" Child A's Mother did not say anything to Ms. Taylor about the dog at the time of the recorded conversation. During my onsite inspection on 4/27/23, I reviewed the allegations with Ms. Kimberly Taylor regarding her dog. Ms. Kimberly Taylor stated that she has an older Chihuahua that can no longer make it up and down the stairs by herself, she's very old with many ailments, unfortunately she will be passing soon. During my inspection the dog was upstairs in the main part of the home. Ms. Kimberly Taylor stated that the dog is not allowed downstairs with the child care children. Ms. Kimberly Taylor explained that the dog sometimes attempts to walk down the stairs where the child care is located but she becomes stuck, there is a gate to enter the basement. Ms. Kimberly Taylor always hears her dog and carries her back upstairs to the main part of the home. Ms. Kimberly Taylor informed me that Child A's Mother toured the child care on a Saturday which is not during child care hours and her dog was also downstairs with her during the tour. Ms. Kimberly Taylor stated, "this is the only time my dog is allowed downstairs" and she assured Child A's Mother that her dog is kept upstairs during child care hours. On 6/23/23, I spoke with Child C's Mother, Child C (female, age 3 years old) has been enrolled in Ms. Kimberly Taylor's childcare for two years. Child C's Mother said she has never seen Ms. Kimberly Taylor's dog around the child care children. On 6/23/23, I spoke with Child D's Mother, Child D (female, age 4 years old) has been enrolled since she was three months old. Child D's Mother said she has never seen Ms. Kimberly Taylor's dog around the child care children.
