LS did not observe all of the required information on each child's form. The Physician information, enrollment, and days and hours of attendance is missing on some of the forms. Provider must have the forms corrected immediately.

Facility is currently advertising that it is a Montessori Program. Facility is not licensed as a Montessori Program. LS observed advertising throughout the facility. The director must remove the term "Montessori" from all advertising and in the facility.
LS observed a bathroom sink that was not operating because the teacher turned the water off so that the children could not use the sink because they play in the sink. The teacher turned the water back on during the inspection. The sink must remain in full operation at all times.
LS observed several classrooms that did not have adequate supervision.
LS did not observe qualified child care teachers assigned to each classroom. Two Aides were covering the infant room.
LS observed 7 infants receiving care in the infant room. The staff stated that a child was moved to the infant room because she was crying. The staff-child ratio must be adhered to at all times.
LS observed one Aide alone with 12 two year olds. During the inspection, the Lead teacher returned to the classroom.
