Violation
During the visit, the following observations were made in the gym area: 1. A mobile partition on wheels was blocking an area with unfinished flooring, exposing the subflooring, which showed signs of staining, damage, and debris. The flooring was uneven and peeling, creating a potential tripping hazard. There was unprotected insulation and concrete, which could lead to moisture and pest entry. Used rolled-up rubber mats were lying on the subfloor, causing dust and debris to accumulate. 2. There is a section along the wall where a water bubbler used to be, leaving an opening that exposes the wall cavity. The opening contains visible piping, surrounded by broken tiles and plaster. This area is within reach of children and presents safety hazards.
