Discussed with the educator the need to secure any openings greater than 3 1/2 inches in the handrail area leading down the stairs to the outdoor play area. The educator was also advised to barricade the side of the corridor/hallway outside to prevent access, and to ensure the fence at the entrance area of the home is at least 4 feet in height. In addition, the educator must remove all rocks and pebbles from the outdoor play area to ensure a safe environment for children.
