Licensor reviewed three random children's files. Two files were found to be up to date with all paperwork. One file was found to have an expired physical. Please check all files to ensure up to date paperwork and acquire a new physical.

Provider stated that her assistant was driving children to school in the morning without proper 7D license and other requirements. Provider is no longer transporting as children are out of school for the summer. Moving forward do not transport children on a regular basis such as to and from school without proper licensing and vehicle requirements in place.

Licensor did a walkthrough of all indoor/outdoor space and found the following hazards: *Bathroom: Child lock was not properly clamped under bathroom sink exposing children to hazards. Please make sure this cabinet is child proofed at all times while children are present. *Basement: Several outlets within reach of children were without covers. Please replace covers. *Backyard: Side of house accessible to children has junk (Provider stated someone was coming to pick it up that week) such as an uncovered grill and a red gas can which present hazards. Provider also has a wired fence around a tree/drop off that is no longer properly linked to another barricade and shows signs of wear making some of the metal points potentially unsafe. Please remove hazards from side of the house and fix the fencing to keep children away from the drop off and safe if they touch the fencing.
