One groups attendance included that there were 16 children present. There were only 14 children present. Two children had left and the groups attendance had not been updated. The attendance for that group was inaccurate.
The program has not been practicing monthly evacuation drills in the Before School Program (BSP). The BSP includes some children who do not attend the afterschool program.

Individual Health Care Plan (IHCP)
A sampling of staff files was reviewed. 1) One staff file did not include sufficent verification of experience and documentation of education to qualify for their assigned position and evidence of 2 doses of MMR or proof of immunity. 2) One staff file did not include evidence and date of an interview and evidence of 2 reference checks.
In the Auditorium, there were 2 exits that did not include a written evacuation plan and evacuation diagram next to these exits.
One child's Individual Health Care Plan and Medication Consent Form included a medication dose of 5 MG (solid). However, the parent provided the medication in liquid form and the conversion dosage from MG to ML (liquid) was unknown.

The Atlantis Classroom attendance included 20 children. The children were counted and it was determined that there were 18 children present. The program must have a method of knowing which children are present in each classroom at any time of the day.

A sampling of staff files were reviewed. 1) One staff file was missing evidence of 2 doses of MMR or proof of immunity. 2) Two staff files did not include current bi-monthly observations.
1) One child's Individual Health Care(IHC) Plan included a 2nd dose of medication should the child still show signs of an allergic reaction after 5 minutes. The parent had provided one dose of the medication. 2) This above child's IHC Plan did not include staff who were trained in medication administration, who trained them and signed/dated by the parent.
This investigation inspection recorded no violations or advisories.

One child's IHC plan included that for a minor allergic reaction a medication should be administered. The parent has not provided this medication and Medication Consent Form to the program.
The Evacuation Drill Log did not include monthly drills for the Before School Care program.

One child's Individual Health Care Plan (IHC) plan from the child's doctor was over one year old. The IHC plan signed by the doctor did not include possible side effects of treatment and consequences if treatment is not administered
Three children's files reviewed did not have current parental consents for children's arrival and departure mode of transportation, confirmation that the child's current physical and immunizations are on file at the child's school and permission regarding publicity and photo's.
One child's emergency medication expired 11/22. In addition, the prescription label (doctor's orders) expired 8/2/22.

Emergency medications were readily available; however, one child's emergency medication had expired 8/20.
The fire drill log did not include a drill in 10/21 or 1/22. Drills must be conducted monthly.

The Green House and Club House classrooms did not include emergency evacuation diagrams and written evacuation plans (including where to meet upon evacuation) posted at each exit.
The program did not have authorization consent for emergency care (First Aid/CPR and transportation to nearest medical facility) for each child easily and readily available and to accompany the children anytime they leave the facility in the care of staff.
