Indoor Space
The indoor space contains hazards, including two curling rugs in the Green Room, shelves are not sturdy and tippable in the Yellow Room and Green Room, a wood box that covers electronic equipment is a hazard in Blue Room 1, and educators' bags were stored in low cubbies and accessible to children in several classrooms.

The yellow classroom had rugs with stains, dirt and crumbs. The tops of the shelves, in the shelves the doors and the walls were dirty with paint, dirt, food, old sticky paper. A shelf in the yellow room was peeling exposing the particle wood. Particle wood crumbles when exposed to moisture and cant be washed, sanitized or disinfected properly.
The allergies, medical information, special diets, emergency medications list posted were not current. Two classrooms had them posted but they were not privacy protected.

The diaper changing tables did not have a posted diaper changing procedure. The diaper changing table in purple room 2 had shoes, cleaning wipes, lotion and wipes stored on it. The diapering mat in purple room 1 had a cloth covering, that was not being washed and disinfected after each use.

The evacuation procedures were missing from the following classrooms exits. Yellow room at main door. Teal room exit to the outdoors and exit into the other room.
An Epi- pen was not in the prescription box with the prescription label. The Benadryl did not have medication consent or prescription on box and it was expired. A mediation administration form was completed for an unknown child in blue room 1.
An IHCP did not have all required information or a parent signature.
The plastic barrier on the toddler/preschool room was worn and falling. The space in-between the straps could be a head entrapment. The preschool classroom had educators purses and bags within reach of the children. While reviewing pictures sent by the licensee, children were in a classroom with tables and chairs stacked on top of the wall and a rolled up rug in the middle of the room. It was reported the room was being broken down to prepare for dance classes.

No peeling or flaking paint (indoor and out)
The door jam in the toddler/preschool classroom had peeling paint within reach of the children.
The classrooms were to dark at rest time, I was not able to see the children's faces or where I was walking. The diaper trash in the yellow room had no lid. The walls and floors throughout the program had paint and dirt on them. The shelves in the blue room had children's materials above the children's heads. The items on the selves could fall off onto a child. The children's cubbies were cluttered and had the children's items spilling onto other children's items and onto the floor. The infant room food prep counter was covered with random items, there was food stains and paint on the counter.

13 children in the blue room with one educator, as the second educator took children out of the room to use the bathroom leaving the classroom out of ratio. The shelves in the blue room are too tall, they create visibility issues when only one educator is in the room.

The preschool room and the new school age classroom had chipping and peeling paint throughout the classroom.
Evacuation procedures posted at each exit
The preschool and school age classroom did not have a variety of materials for the children to choose from.There was not a variety of interest areas set up.
The emergency evacuation procedure is missing at the exit in the school age classroom.
A infant was observed laying in their crib drinking their bottle.
A infant was drinking their bottle laying down in their crib, the educator was told the infant needed to be held while eating. The educator took the bottle from the baby and walked away. The baby was crying in the crib.
Both gates on the playgrounds had head entrapments. The chain link was not properly attached on the top or bottom. The toddler playground had adult size picnic tables and a pop tent that was not fully built or secured to anything.
A infant was left in a stroller outside of the infant room. One educator was doing diapers out of view from the infant in the stroller, while the other educator was working with infants in the room.
The staff educator schedule posted was not current. Four teachers qualifications were reviewed, the four teachers reviewed did not have evidence of being certified or qualified teachers.

There was no evidence that 4 staff had EEC essentials trainings. There was no evidence that the infant room educators had SIDS training prior to working in the infant room. There was no evidence in the staff files that a EEC program orientation was completed.

Physical Space Paint
On the day of the visit, the following classroom items were identified as potential hazards: 1) Several garbage cans did not have covers.2) Several classrooms had electrical cords, longer than 6 inches, that were hanging in areas accessible to children. 3) there was excess clutter on top of cubbies in the Yellow Room. 4)There was a toilet bowl cleaner accessible to children in the Yellow Room bathroom. 5) The freezer in the Yellow Room was dirty. 6)The floor in the bathroom adjacent to the Teal Room, had a crack in the floor.
On the day of the visit, one child's prescription medications was not in the container in which they were originally dispensed and with their original labels affixed.
