An infant was observed being placed on the floor to play immediately after finishing their lunch without first washing their hands.

The following was observed: * Stained ceiling tiles in the preschool classroom and bathroom *Baseboards in the hallways peeling off the walls. * A cardboard slide was observed being used by an infant without the required mat surrounding it. *A tall doll house was not secured to prevent tipping.
A medication consent from for an over-the-counter cough medicine for an infant did not contain the required physician's signature.

An infant was observed sitting in a bouncy chair feeding their self a bottle.
An educator was observed alone with five toddlers.
Three toddlers were observed alone in a classroom while the teacher was in an adjacent classroom out of sight from the children.
The infant/toddler refrigerator did not have a thermometer.

Through a review of emergency medications, a [REDACTED] for Child A did not have current [REDACTED] and the medicine had expired.
The 2nd-floor stairwell which brings the children to the gross motor activity space was not protected as both the hallway door and the gross motor area door were not closed during the time preschool children were using this room.

When the Licensing Specialist arrived the preschool classroom of children was just finishing up using the upstairs, hall for indoor gross motor time. A child of this grouping, was using the bathroom around the corner from the hall unsupervised. The 2nd floor area is an area of the program that the public has access to.
At the time of the visit, the child size soft couch in the preschool classroom was not in good repair, as the covering to this couch was beginning to tear. Also, rows of adult sized chairs stacked five chairs high, and along one entire wall the of the 2nd floor hall that the children use for indoor gross motor, were activities were not protected in a manner that eliminated the risk of the chairs falling on a child if they were bumped into.
