1) Two medications were not readily available at the time of the visit. 2) EEC observed a child's {REDACTED} inside an {REDACTED} medication box. The {REDACTED} and the medication box contained different lot numbers and expiration dates. 3) A child's {REDACTED} was not in the original packaging in which it was originally dispensed, and it did not contain the original affixed label. Please ensure that all prescription medications are in the containers in which they were originally dispensed and with their original labels affixed. Over-the-counter medications must be in the original manufacturer's packaging.

The children's record review reflected the following: 1) Several records did not contain the date of admission. 2)Two children's records did not contain a transportation plan. 3) Two children's records did not contain annual physicals. 4) Three children's records did not contain evidence of {REDACTED}.
The staff record review reflected the following: 1) A staff record contains no evidence of current physical and {REDACTED} immunity. 2) A staff record did not contain sufficient evidence of {REDACTED} immunity. 3) A staff record contains no evidence of completing the EEC Essential Modules (12hrs). 4) Two staff records contain no evidence of periodic bi-monthly observations.
The individual health care plan review reflected the following: 1) Five individual health care plans missing parent signatures and one of these plans was also missing the physician's signature. 2) A child's individual health care plan was not available for review.
A child's record contains no evidence of medication consent form for an over-the-counter medication.
