Furthermore, there was insufficient documentation of bi-monthly observations. 2) Educator {REDACTED}’s record contained no evidence of ongoing bi-monthly observations. 3) Educator {REDACTED}’s record contained no evidence of MMR immunity and ongoing bi-monthly observations.

At the time of the visit, the building inspection was expired, 8/2024.
The children's record reflected four records containing no evidence of progress reports.
The children's record checklist was not updated/current.
The records of two staff members did not contain evidence of current first aid certificate.
A background record check review reflected the following: 1) Three staff members with expired background record check.
1) The staff schedule was not current and was not posted in an area easily visible to educators, parents and visitors. 2) A staff member was caring for ten children in an unlicensed space with no evidence of group leader qualifications on file.
At the time of the visit, childcare was taking place in the art classroom, space not approved by the Department. There were 10 children in the classroom.
At the time of the visit, the backup snacks were expired, October 2023.
Three individual health care plans did not include a parent signature.

Hazardous materials (Clorox wipes, unlabeled spray cleaning product) were placed on top of a shelf in the cafeteria, well within reach of children.

Four staff were observed gathered near the entrance of the building conversating while children played in various areas of the playground.

On the date of the visit the Building Inspection had expired 8/27/2021. The Fire Inspection dated 8/27/2019 and expired 8/27/2020.
The time of arrival was not documented on the daily attendance for four groups.
