The Building Inspection certificate expired on 9/21/2019. The Fire Inspection certificate expired on 10/11/2019. Every program that is not located in a residence and every program that has a licensed capacity of more than 10 children must provide a certificate of inspection from the Department of Public Safety or the local building inspector certifying that the facility complies with the State Building Code (780 CMR). Every program not located in a residence must submit evidence of compliance with applicable fire codes.
Emergency/evacuation procedures were not posted at two exits of the preschool classrooms. Next to each exit, emergency and evacuation procedures must be posted.
In the Preschool transitional room, (1) child’s {REDACTED} had an expiration date of 4/2019. When possible, all unused, discontinued or outdated prescription medications shall be returned to the parent and such return shall be documented in the child’s record. When return to the parent is not possible or practical, such prescription medications must be destroyed and the destruction recorded by a manager or supervisor in accordance with policies of the licensee and the Department of Public Health, Drug Control Program.
