Licensor reviewed 3 of the children's progress reports. Of these, 2 were complete and up to date, the 3rd report was out of date.
All of the required items were observed in the 1st aid kit except for tweezers.
Licensor randomly reviewed 5 of the children's progress rpeorts. Of these, 3 were complete and up to date. For the other 2 reports, Educator stated they were complete, but there was not a copy in the children's records for Licensor to verify this.

The temperature in the refrigerator was 30F degrees, which is a little too cold as regulations state the temperature must be between 32F-40F degrees.

Licensor randomly reviewed 5 of the children's records. Of these, 3 were complete and up to date, 1 had out of date signatures in the enrollment packet, and the last had no evidence of lead testing.
According to the Evacuation log, Educator has not conducted a fire drill since 4/10/23.
All of the required items were observed in the first aid kit, except tweezers and medical tape.
Educator utilizes a changing pad for diapering. This pad had a cloth cover over it. Discussed that this cloth cover cannot be disinfected after each use and therefor cannot be used.
Educator utilizes a fenced in area in the side/back yard for the children's outdoor playspace. This area is completely covered with pea stone. Educator also has a side area inside of a play yard for children under 2. During the upgrade visit, there were children under 2 years of age playing in the pea stone area. Pea stone is considered a choking hazard for children under 2.

This monitoring visit inspection recorded no violations or advisories.
Educator tracks the attendance in a planner notebook. The time of arrival is noted for each child daily. However, Educator has not been tracking the time of departure. Discuss that this is a regulation and this information needs to be added.

Educator has a detailed emergency preparedness plan posted on a bulletin board in the playroom. However, it does not include the plan for if a child goes missing.

Educator has an Evacuation log that has completed fire drills for every month except November. The last drill completed was on 10/11/19.
Educator does not brush the children’s teeth. There are signed Oral Health Non-Participation forms in all of the children’s records. However, these forms need to be signed annually and some of them are out of date.
