At time of visit there were 8 children present, four children under the age of 2 years old with one educator present and one unapproved caregiver. Educator indicated approved assistant left around 10:30am due to an emergency. Educator called assistant to return to daycare, assistant returned while licensor were still present. It was observed that there were three children that were being restrained. Two children were observed to be restrained in bouncy chairs which were not developmentally appropriate, one child was (REDACTED). There was an (REDACTED)old in a pack and play who was awake. The children were observed to be left in these furnishings for an extended period of time, well over 20 minutes. Licensor had educator remove the children from these furnishings. There was back and forth conversation explaining to the educator why keeping children in bouncy chairs and pack and plays for an extended period of time was not appropriate.

It was observed that in the pack and play where an (REDACTED) infant, as stated by the educator had been napping, had an empty bottle, a bib, and a soft toy inside the pack and play all of which are violations of the safe sleep policy. Additionally there was another pack and play where a (REDACTED) infant naps, this infant was not in the pack and play at time of visit; it was observed that there was a rolled up blanket, a soft toy, and had the attached changing table which had diapering materials on top as well as a large boppy pillow on top, these are also violations of the safe sleep policy. Upon review of educator’s licensing history, it was found that on June 24, 2014 the licensor observed an infant fall asleep in the baby swing. Therefore, this is considered the educator’s second safe sleep violation. Licensor will review this information with the FCC supervisor. Licensor will email safe sleep parent letter to both parents of these infants, educator will need to read and sign safe sleep pledge, the educator will be required to re-take the safe sleep training online, and licensors will conduct a follow up visit. Licensor will follow up with educator for any further action required.
