At the time of the visit, in a review of four students files, the documentation was not fully complete. Three students had an expired physical. One student was missing documentation of a physical and their immunizations. Children's records must have all required applicable documentation according to the Children's Record Checklist.

At the time of the visit, in a review of four educators files, the documentation was not fully complete. All four educators had expired staff observations. Two educators were missing documentation of two references. Staff A had an expired physical. Staff C had an expired PQR and was missing documentation of date of interview and their program orientation. Staff D was missing documentation of application/ resume, documentation of a physical and their MMRs. The program must maintain a personnel record for each staff member, as outlined by the staff record checklist.

At the time of the visit it was identified that one educator on site had an expired BRC. The employees BRC had been expired for two and a half weeks. The licensee must maintain in each personnel record: evidence of completed background record checks, as required by 102 CMR 1.05(2) and 606 CMR 14.00.

Violation

At the time of the visit, the following was observed: thirteen students were outside on the Infant/Toddler playground with two educators. The thirteen students were part of the Toddler Classroom and some of the Infant/Toddler Classroom. The attendance record did not accurately identify the Infant/Toddler students who were outside. When the Infant/Toddler group went inside, the teachers placed the students in an area of the classroom separated with a gate. The two teachers were on the opposite side of the gate, from the students. This resulted in the students not being properly supervised. When EEC identified this concern, the Licensee removed part of the gate, to have the area fully accessible. An educator within the Toddler/ Preschool Classroom was observed to leave the room, leaving one educator alone with nine students. The licensee must maintain sufficient numbers of qualified staff to promote the health, safety, growth and development of each child. Assignment of staff must take into account the physical environment, requirements of the activities children are engaged in, and the developmental levels and behavioral traits of children in care. The program must have the number of educators necessary to: provide individual attention to children; and ensure adequate supervision of the group at all times.

At the time of the visit, an educator within the Infant/Toddler classroom was observed to warm a bottle within a microwave. Liquids, foods, and appliances that are or become hot enough to burn a child must be kept out of the reach of children. The licensee must ensure that snacks and meals are prepared and served in a safe and sanitary manner. Per the Bottle Warming Safety Policy, Bottles may not be warmed using bottle warmers that use water and/or steam, microwave ovens, or crock-pot/slow cookers.

Violation

At the time of the visit and in a review of the programs document library, the Building Inspection and Fire Inspection were expired. Every program that is not located in a residence and every program that has a licensed capacity of more than 10 children must provide a certificate of inspection from the Department of Public Safety or the local building inspector certifying that the facility complies with the State Building Code (780 CMR). Every program not located in a residence must submit evidence of compliance with applicable fire codes.

Violation

At the time of the visit, it was observed the Infant/Toddler playground had a play kitchen which was not properly secured and was tippable. The licensee must only use indoor and outdoor equipment, materials, furnishings, toys, and games that are appropriate to the ages, needs and developmental level of the children enrolled. They must be sturdy, safely constructed and installed and non-tippable.
