In the playroom a small portable space heater was observed to be on the bookshelf that needed to be removed. One of the bookshelves in the playroom was not secured and was a tipping hazard. There was no gate secured at the bottom of the stairs securing the stairs from the children. One of the trash cans in the bathroom was uncovered. The outdoor play space is gated from the rest of the yard. The gate's latch was broken and not able to be secured allowing access to the remainder of the yard. There were various hazards viewed in the yard space including but not limited to the following; a gas grilled which was uncovered, broken toys, broken wood, a ladder which was open and able to be climbed, screws, cigarettes, a lighter, trash, cinder blocks. peeling and chipped paint on the shed and an open falling area under the deck.
