Individual Health Care Plan (IHCP)
At the time of the visit, and in a review of the document library, it was identified that the program had an expired Building Inspection. The inspection expired in October 2025. The Building Inspector arrived at the program to complete their inspection while EEC was at the program. Every program that is not located in a residence and every program that has a licensed capacity of more than 10 children must provide a certificate of inspection from the Department of Public Safety or the local building inspector certifying that the facility complies with the State Building Code (780 CMR).

At the time of the visit, in a review of four educators files, the documentation was not fully complete. Two educators were missing documentation of two MMR Vaccines and professional development. Two educators had expired PQRs, expired annual evaluations, and expired observations. One educator was missing documentation of a staff form, a physical, PQR and observations and one staff member had an expired CPR certificate. The program must maintain a personnel record for each staff member as outlined by the staff record checklist.
At the time of the visit, it was observed that an Assistant Teacher took students to use the bathroom. The bathroom was located outside of the classroom, upstairs, within another classroom space used by the program. For purposes of calculating staff to child ratios, staff shall include only those EEC-qualified persons who are assigned to care for children for specific hours of the day.
At the time of the visit, it was observed that the downstairs preschool classroom had eleven students present. The classroom measures for a maximum of ten students. The licensee must provide a minimum of 35 square feet of activity space per child.
At the time of the visit, it was observed that the outdoor area had three slides that were not six feet apart. The use zone should extend a minimum of 6 feet in all directions from the perimeter of any stand alone climbing structure.
At the time of the visit, in a review of one students IHCP and the corresponding prescribed medication, not all medication was one site. Student A was missing their prescribed medication for their mild symptoms. Emergency or life-saving medications, for any children for whom they have been prescribed, must be readily available.
At the time of the visit, in a review of one students IHCP, the documentation was not fully complete. Documentation of the educators trained on the health condition was missing. The licensee must maintain as part of a child's record, an individual health care plan for each child with a chronic medical condition, which has been diagnosed by a licensed health care practitioner.
At the time of the visit, in a review of one students IHCP and the corresponding medication consent documentation, the documentation was not fully complete. One student had two prescribed medications. They were missing medication consent for one of their prescribed medications. Their second medication consent document was missing documentation of the potential side effects of the medication and the directions for storage.

At the time of the visit, in a review of two students IHCP's, the documentation was not fully complete. One student was missing the symptoms of their health condition. Another student was missing documentation of the educators trained on the health condition. The licensee must maintain as part of a child's record, an individual health care plan for each child with a chronic medical condition, which has been diagnosed by a licensed health care practitioner. The plan must describe the chronic condition, its symptoms, any medical treatment that may be necessary while the child is in care, the potential side effects of that treatment, and the potential consequences to the child's health if the treatment is not administered.

At the time of the visit, within the classroom on the middle floor, it was observed that an educator walked into the kitchen space (which is separate from the classroom). The kitchen space had a half door and a "cut out" within the wall that looked into the classroom. The educator was using the kitchen to get warm soapy water to clean the tables. Due to the kitchen space being separate from the classroom, the educators cannot enter while they are considered in ratio within the classroom. Educators must not engage in any other activities or tasks that could unnecessarily divert their attention from supervising the children. Educators must exercise appropriate supervision of the children in their care
At the time of the visit, in a review of the evacuation drill log, there was not documentation of April, July and August 2024 evacuation drills. The educator must hold practice evacuation drills with all groups of children and all educators from each floor level of the approved space at least monthly. Drills must be held during different times of the program day, and must use alternative exits. The educator must document the date, time, exit route used, number of children evacuated and effectiveness of each drill.
At the time of the visit, in a review of three prescribed medications associated with two IHCP's, one student had an EpiPen that was not in the original packaging with the prescription label attached. All prescription medications must be in the containers in which they were originally dispensed and with their original labels affixed. Over-the-counter medications must be in the original manufacturer's packaging.
At the time of the visit, it was observed that the tables were being cleaned using a sponge. Per EEC "cleaning, sanitizing and disinfecting surfaces in Child Care Setting" policy, Sponges must not be used for sanitizing or disinfecting.
At the time of the visit, in a review of two IHCP's and the corresponding medication consent documentation, the documentation was not fully complete. All medication consent documentation reviewed was missing if the child had taken the medication before, the dates for the medication to be given, the times for the medication to be given, the possible side effects of the medication and the directions for storage.

A review of the evacuation drill log indicated that a drill was not conducted on a monthly basis during the 2021-2022 school year.
The following was observed and accessible to children: -In the Dramatic Play room, the small plastic tool bench and three play kitchens were not secured and tippable. -On the main floor, there was a plastic tool bench and white shelves in the Block area that were not secured and tippable.
A thermometer was not located in the refridgerator.

A review of four staff records indicated the following: -All four did not have evidence of Strong start Essentials trainings completed. -All four did not have evidence of bi-monthly observations completed. -One did not have a current PQR registration. -One did not have evidence of a current physical and immunizations. -One did not have evidence of a completed program orientation.
The attendance record located on the main floor does not include arrival and departure times.
Equipment is clean, safe, secured, and poses no hazard.
A review of three staff records indicated that all three did not have evidence of bi-monthly observations or staff qualifications. One file did not have evidence of a current registration renewed annually.
A review of the evacuation drill log indicated that a drill was not conducted in the months of May 2021, June 2021, November 2021, December 2021, January 2022 and March 2022.
It was observed that the play kitchen on the main floor and the tool bench on the second floor were not secured and tippable.
An evacuation procedure was not posted near the main entrance and downstairs side exit door.

After a review of three staff files, it was observed that all files were missing required annual training certificates for 2019.
It was observed that: -in the nap room/music room upstairs, there was a shelf that was tippable. -in the art/lunch room, the shelf next to the water cooler was tippable. -the shelf next to the child bathrooms was tippable.
Fire Inspection is not current, facility was in violation of two light fixtures that need to be replaced
After a documentation review of two children with medication, it was observed that: -one child had an expired Medication Action Plan with no parent signature and was missing a signed medical consent form
