Records were reviewed for the 7 enrolled children. One child needed an Annual Update.
The first aid kit was missing emergency cards for 2 enrolled children. There were 2 instant cold packs that had popped.
Progress reports were reviewed for the 7 enrolled children. Five of the children did not have current progress reports.
There were disposable gloves, diaper cream, plastic from bags of diapers, and wipes packages accessible to children near the changing table. In the kitchen there were drawers and cabinets that had hazards accessible to the children (pizza cutter, electrical cords longer than 6”, a lighter, cleaning products, etc.)
The smoke detectors were tested 3/4/2026. The smoke detector on the first floor was not working.
One child did not have their lead testing or an annual update form.
The educator reported a family member has been residing in the home since January 2024. The household member has not completed the background check process.
The fire alarms were tested and not functioning on the first floor or in the basement.
In the bathroom, there were no paper towels for hand drying accessible to the children.
There educator did not inform EEC in the lead portal of the change in household composition.
The exit to the front exit did not have an exit sign posted.

The educator is not documenting the time in/time out of their assistant's working hours.
Licensed Space
During this visit, developmentally appropriate curriculum was not observed. Children were watching television for an extended period of time and three of the children were confined to highchairs for an extended period of time with no appropriate activities.
Progress reports were not current for all enrolled children.
The smoke detectors on the first floor were not working. They were working on the lower level.
The children were in the living room, adjacent to the kitchen on the upper level, when Licensor arrived. This room is not licensed space, and there were hazards accessible to the children in that room (uncovered outlets, loose electrical cords, a shelf that was a tipping hazard, plastic bags, etc.) The Educator was making lunch in the kitchen, which is licensed space. The Educator stated she brought the children upstairs to supervise them while she made lunch. The Certified Assistant was no present when Licensor first arrived, so there was no one available to stay downstairs with the children while lunch was being prepared. Discussed the children can only be in licensed space and there needs to be a better plan for preparing lunch if only one caregiver is present.
Three toddler-aged children were observed to be confined to highchairs for an extended period of time.

Equipment and Materials
There was not enough materials and equipment accessible to the children when Licensor first arrived. There was only one bucket of dinosaurs and a few other toys (rubber duck, car, etc.) available to children.

The Certified Assistant doesn’t have the required amount of professional development training hours.
There were large branches on the ground in the fenced outdoor play space. The area under the deck was also accessible to children.
The Educator did not have proof of 30 hours of professional development training hours. She had certificates showing 20 hours of trainings. Please send proof of additional training hours.
This monitoring visit inspection recorded no violations or advisories.
