One child needs a current lead screening test and medical exam.
Two children need current progress reports.
No daily attendance available at the time of the visit. Please submit copies for January 2025 to current date.
Need adhesive tape, gloves and gauze roll for the first aid kit.
The water temp is reading 144.6 degrees, it must not exceed 120 degrees.
There is play ground equipment that would requires appropriate fall zone materials. The play structure needs additional fall zone material in front and back. Licensor will send Home Playground Book.
Need to add current training hours to PQRegistry, the last training hours added is from 2022.
The refrigerator temp is not reading correctly. It must read between 32-40 degrees.
Need fitted sheets for the playpens. The infant was not sleeping during the visit but infant safe sleep practices were discussed. There is a playpen set up for the infant that is present.

Two children need lead screening tests. One child needs immunizations. One child needs a current medical exam.
Two children need current progress reports and progress reports are not being completed as required by the regulations. Progress reports must be completed every three months for infants until they reach fifteen months, every six months for toddlers and preschool children and once a year for school-age children. A child with special needs must have a progress report completed every three months.
Need to add training certificates on the PQRegistry under professional development. Renewal date for the PQR is 5/3/2025.
The bathroom window from the bottom up is more than four inches in height.
There needs to be additional age appropriate materials for the children to play with such as big Legos and puzzles etc.
Observed a diaper change, hands must be washed after each diaper change and the changing area cleaned.

Put secured lock on the sliding door in the basement that has pipping. Need a barrier to prevent access to the hot water heater etc in the basement.
Peeling paint on inside of hatchway doors.
Remove cement blocks.
Need a padlock for swimming pool deck gate. Need an expansion gate at the bottom of the pool when children are present.
