Three children need their enrollment forms updated. Three children need current medical exams. One child needs immunizations. Two children need current lead screening test. The assistant needs to be added to parent permission forms for [Redacted]. Two parents need to sign and date acknowledgement of receiving the parent handbook.
The hot water temp exceeds 120 degrees, it is reading 131.1 degrees.
The basement is not meeting regulations to be used with the children. The educator will be submitting an transaction to remove the basement until it is ready to be used by the children. The outdoor play space needs to be cleaned up of any broken and worn equipment and play materials. There is a gate to prevent the children access to the kitchen exit to exit.

The open garage is accessible to the children when playing in the backyard. Reminder the water hose must be put away to prevent tripping.
The fabric platform swing without safety belt must be removed, not allowed to be used with the children. The other two swings need to be spaced farther apart.

The basement has some wall repairs needed before the children can use it. The educator continues to work on the space rearranging and changing materials.
The back porch screen door needs to be repaired.

There is a swing hammock in the children's play space that need to be removed. The basement space has to be incompliance when the children are present for care. Organize the basement space there is a ladder, a broken ceiling fan in a box at the bottom of the stairs that needs to be inaccessible to the children.
Need to complete progress reports for six children. All children under 15 months must have progress reports completed every three months, children 15 months up to school-age must have progress reports completed every six months.
A review of enrollment forms shows missing information such as physical description, age at admission, date of admission, date of signature and assistant name on written parent permission forms on some of the children's files please check them all. Four children need current lead screening tests. Three children need current medical exams.

We discussed organizing the indoor licensed space and materials which includes the kitchen, and the playrooms. We discussed organizing the outdoor space and making the garage inaccessible to the children. The hatchway door in the basement needs to be cleaned of dust and webs.

Submit a picture of the kitchen cleaned up and decluttered by 6/5/23. Submit a plan to make sure the space stays incompliance with the regulations. The kitchen has to be decluttered Counter tops, drawers can't close and hazard tools removed such as downy, a hand saw and an electric saw. The dish washer opened filled with dishes, and the trash can full and trash accessible.
Four children needs up dated progress reports. Two are school-age and two are under four.
Need to keep a log of days and hours the assistant is working at the program. The assistant does not have to work at the program if the provider falls within the capacity to work

The licensed indoor space first floor and basement need to be cleaned and cleared of bags, bins etc. which is limiting the useable space for up to ten children.
There are some outside play structures that are not in good repair, needs to be removed.

Need medical form, immunizations and lead screening test for two children. Need to add assistant's name on all written parent permission forms.
Need a plan for keeping the children's play space free of dog feces. The immunizations for the dog are available.
Need a working carbon monoxide detector for the second floor.
