The following was identified as concerns: Pink Room: the wall above the sink in the bathroom was dirty, the trash can was dirty, the floors and base molding were dirty. The walls were dirty with debris that look like it dripped and was never cleaned. There was an ottoman that was in poor condition. There were several holes in the walls. A toilet cleaning brush was in the bathroom and accessible to children. Blue Room: There was garbage on a windowsill, shelves that were not secure and pose a tipping risk to children, closet was unlocked with cleaning supplies. There were brown boxes covering an item that stated caution risk of electrical shock and could easily be moved causing access to the children. Paint in the room was peeling/chipping, long cables were accessible to children. There was a tall white shelf that had exposed nails. Green Room: The windowsills were dirty with dust and debris, the blinds were broken, floors were dirty with dirt and gum, there were cables that were not secured, the tall brown shelf was not secure, there were chemicals accessible to children in the closet that was not locked, couches were in poor repair and could not be cleaned or sanitized. There was no cover on the trash. There were brown boxes covering an item that stated caution risk of electrical shock and could easily be moved causing access to the children. Taekwondo Room: the mats are dirty, there was a large hole in the wall, there was dirt and debris on baseboard and around all doors. The bathroom was dirty with dirty paper towels on the floor. Kitchen: the kitchen was accessible to children there was no door closed or locked to prevent children from accessing it. In the kitchen all cabinets were sticky, the microwaves were dirty, in a drawer there was a lighter, under the sink there were chemicals that were accessible to children. Hallway: All halls were dirty; walls had peeling paint. In one hallway there was a utility closet that was wide open and accessible to the children that had hazardous materials in it. In two classrooms there was no toilet paper in the bathrooms.
