The educator was not always supervising and did not see when a child was pushed by another child. The same two children were later fighting and the educator did not interfere. The educator left the infant on the diaper changing table.

The following were observed during the visit: Chipping paint on the wall between the living room and playroom and the doorframe between the seasonal porch and playroom. There was an unsecured gate at the bottom of the stairs (this was previously cited). Cleaning solution and toilet brush within the reach of children in the bathroom. The kitchen radiator has a cover that was open allowing the children access to it.
