Licensor observed a child inside a playpen with a bottle. Provider reported that this child still drinking milk in a bottle due to his developmental delays. Licensor told provider that this child must been fed while at a child's size table or a high chair, but never in a playpen. Provider belongs to Providers food program.

Upon arrival, Licensor observed that there were two children on high chairs on the hallway to the bedrooms. According to the provider, these children have finished eating. Please understand that you must provide sufficient space for the children to eat in an un-crowded manner and to meet the needs of all children. Also this hallway could not be blocked at any time during child care hours. The home temperature in the licensed areas and more specific in the kitchen area was too cold. Upon arrival, Licensor observed in the kitchen, an 8 month old child in an infant feeding chair, where the room temperature tested by this Licensor was 66 degrees F. The recommended room temperatures when caring for an infant must be between 68 to 72 degrees F.

During this visit Licensor observed that there was snow on side porch which faces the side exit of this home. Exits must be clear of snow during child care hours to avoid children’s injuries while coming in or while exiting the home.

During this visit Licensor observed that part of the kitchen ceiling that faces the side exit have collapsed. This area was partially covered by plastic exposing a health hazard to the children in care. Provider at first reported to this Licensor that this part of the ceiling collapsed two days ago caused by a water leak since her home’s roof was not properly sealed. Then the provider said that the area where the ceiling collapsed happened a week or so ago from the date of this visit after the roof was properly sealed, but she decided to keep doing care. Provider admitted that she was aware that her day care could it be closed with this damaged ceilings. Due to this issue this Licensor told the provider To close her program on the date of this visir until the ceiling was fully repaired by a licensed contractor. Licensor assisted provider calling Bethel FCC so they could call the children's parents to pick them up. Also, Licensor told provider to contact this Licensor immediately after this repairs were done to conduct a follow-up visit to determine that the corrections were done and that she was approved to re-open her program.

Upon this Licensor's arrival the provider reported that a child had locked herself in the bathroom. This child was crying inside the bathroom and almost every child in care was crying as well. The provider was holding the skeleton key to open the bathroom door and reported that she had tried to open it unsuccessfully. This Licensor was going to try to open the door when suddenly the provider was able to open this bathroom door. Licensor observed that the light in the bathroom was off and that she child stopped crying immediately after the door was open. Provider reported that this child have lock herself in the bathroom before. The provider showed to this Licensor where she stores this key which is on a kitchen wall closer to this bathroom and out of the reach of the children. Technical assistance was given to the provider to change the bathroom door knob to one that has no inside lock to prevent these incidents to happen again. Please understand that when these incidents have happened you have taken the direct supervision away from the other children that are in care.
