Emergency medications were not readily available, the IHCP called for a second epi pen and there was not a second epi avaible for two children.

There were children that did not have IHCP at the time of the visit. Other IHCP were incomplete.

In the gym there were wires that were accessible to the children. It was observed that the bathrooms appeared to not be cleaned in days. There were rings inside the toilets. There was a very strong smell of urine. The administrator reported that children and parents have complained about the lack of cleanliness in the bathroom.
