Please ensure to obtain an up to date Individual Health Care Plan as soon as possible and ensure all Individual Health Care Plans have written parental consent with authorization from the health care practitioner.
The personal belongings of staff were within reach of children in some of the classrooms. Please ensure all hazardous objects, including but not limited to matches, lighters, toxic materials, sharp objects, plastic bags, and purses are locked or inaccessible to children.

Please ensure all unanticipated non-prescriptions prescribed in an Individual Health Care Plan are on site at all times and ensure to obtain an up to date unanticipated non-prescription.

Please ensure to obtain an up to date Individual Health Care Plan as soon as possible and ensure all Individual Health Care Plans include authorization from the health care practitioner and parent.
An emergency medication was not stored in its original container. Additionally, an unanticipated non-prescription for mild symptoms that was prescribed as part of a child's Individual Health Care Plan was not on site. Please ensure all prescription medications are in the containers in which they were originally dispensed with their original labels affixed and ensure all unanticipated non-prescriptions prescribed in an Individual Health Care Plan are on site at all times.

In the downstairs bathroom where the children wash their hands there was a Lysol spray and powder cleaner accessible and within reach of the children.
This monitoring visit inspection recorded no violations or advisories.
Several of the children's records did not have a First Aid and Emergency Medical Consent form.

The following documentation was missing and/or expired from the staff files: (4) PQ registry missing, (1) missing {REDACTED} results, and (1) CPR expired.
The snacks being stored in covered containers did not have an expiration date listed on the package.
At the time of the visit the following was accessible and within reach of the children: (2) Clorox wipes containers and (2) spray cleaning bottles.
The Evacuation Practice Fire Drill was missing documentation for the exit used and the effectiveness of drill.
The educator did not wash their hands prior to putting a food glove on one hand then using the other hand to grasp the other hand contaminating the glove. The table was not washed and disinfected after the children ate their snack and before starting the baking activity.

The following documentation was missing from the staff files: (1) reference was missing, the School Work Verification experience form needs to be completed for several staff, (1) physical was missing, and (3) {REDACTED}results was missing.
In several of the children's files the following documentation was missing: Missing First Aid and Emergency Medical Consent form, permission for photos, and permission for off site walking.
The Staff Schedule was posted in the Directors office however it needs to be posted in the program for parents, volunteers, and staff to view.
At the time of the visit (1) {REDACTED} was expired.
The following documentation was missing from the Individual health Care Plan: (3) IHCP did not have the side effects, consequences, who is trained to administer the life-saving medication and who trained the individuals to administer the life-saving medication, and (1) Individual Health Care Plan did not have a parent signature with the date.
