The following were observed: -Gloves and cords over 6 inches accessible to children.
Emergnecy evacuation observations did not include the number of children evacuated or the time of the drill (consistently).
Gloves were accessible in one room. One large vent near the floor was dusty and another was rusted, in the infant space.

An (REDACTED) infant was observed sleeping on a cot, with a thick blanket covering (REDACTED).
A review of transportation records for the last week indicated that children on the morning transportation run were transported for more than 45 minutes. The longest time noted that a child was on the bus was 5:46 am until 7:12 am. Children must not be regularly transported for periods longer than 45 minutes one way between their home or school and the program.
