Progress Reports
Data synced from Massachusetts Department of Early Education and Care on Jul 10, 2026 · Source records · Report an error
Educator stated that she emailed the completed progress reports to the parents in march, but did not have any copies to show to Licensor during the visit. Licensor asked for Educator to email the completed reports for verification. However, Licensor did not receive an email from Educator and therefor was not able to verify that the progress reports had been completed.
Licensor inspected all of the licensed, indoor space during the monitoring visit, as well as the fenced in space in the back yard. The following hazards were observed: **PLAYROOM 2** - outlets not covered **PLAYROOM 1** - outlet not covered **BATHROOM** - no hazards **OUTSIDE** - peeling paint on the railings and steps of both decks in the back yard.
Educator's CPR/1st Aid certification was issued through the International CPR Institute, on 9/18/22. This certification appears to be completely online, with no hands on skills demonstration portion. This is against the current EEC policy, which has been in place since it was last updated on 12/17/21. The policy states that online CPR training is only acceptable if there is a hands on skills demonstration section. EEC contacted the International CPR Institute through email and they confirmed that there is no hands on skills component to their training. Educator will need to become recertified in CPR/1st Aid within the next 7 days.
Educator is being cited for supervision concerns due to the fact that she was alone with 9 children when Licensor and Supervisor Faulkner arrived at her program.

Space (indoor / outdoor hazards) - R Apr 2025
When Licensor and Supervisor Faulkner arrived, there was 1 adult (Educator) and 9 children present. Educator stated her assistant had to leave briefly, due to a personal situation. Approximately 20 minutes later, the assistant arrived at the program.
Licensor reviewed 5 of the children's records. Of these, 3 were complete and up to date, while the other 2 both had out of date medical information. Please review all of the children's records and ensure they are all complete and up to date.
Licensor inspected all of the licensed, indoor space during the progress visit, as well as the outdoor playspace. No hazards were observed in any of the licensed space inside. The outdoor playspace is a fenced in area in the back yard. There are multiple pieces of playground equipment that require a fallzone. However, the area is covered in packed dirt that is not an absorbent material. A fallzone also is required to extend 6 feet out in every direction and there is not enough space for that right now, due to the proximity of the fence.
Staff Records
Educator has a record for her assistant. Her assistant did trainings complete any professional development hours for 2021 or 2022. Over the last 3 years, she has 18 hours of professional development, but 30 hours are required.
Outdoor Playspace - Hazards
The only outdoor hazard that was observed was a shelving unit in the shed, which is a tipping hazard.
