Indoor Space
During the observation of the infant toddler room there was a highchair on a heater that was accessible to children and could fall on them. There was a fair amount of clutter in the infant toddler room, specifically in the windows and around the blocked off teacher space. In the teacher space there was a strap to a purse that was reachable by children which could cause the purse to be accessible to children. There was also an extension cord that had the potential to be reached by children.

Documentation of medication administration; Current consent for medications
A child with asthma did not have a current individual healthcare plan, the educators were not knowledgeable about the status of the child's medication. One educator reported the child did not have the medication on site, upon review of the medications the Licensor found the medication in the medication box in the classroom, but there was no prescription label or consent for the medication.

Staff Meetings were documented for every other month. The Director reported 2 hour staff meetings are held every other month since covid.
In the infant/toddler room one of the educators was not responsive to the children when they needed guidance, intervention, behavior support and self-regulation. Another educator in the classroom was observed forcing two children to make handprints with paint. The two children were crying and pulling their hand away when the educator was painting their hand. The educator continued to put paint on their hand and make the print on the paper.
While observing the infant/toddler classroom there was an educator that did not engage with all children. They sat in a chair with one child, while the other educator was doing an art activity, washing up the children, redirecting them and trying to support their individual needs. At one point a toddler was left in the bathroom after washing their hands, and after being called several times the child did not come out. The educator working with the children had to go get the child, while the educator in the chair never stepped in to help. The educator working with the children on the art activity began to show signs of frustration through her tone and language used.
A child with asthma in the preschool classroom was not listed on the Allergies, medical information, special diets, emergency medications list.
The Albuterol on site did not have a medication container or label.
There is not a current medication consent form for albuterol in the preschool classroom.
The changing table had multiple packs of wipes, a sippy cup and cloth toy on it.
The toddler classroom had two glider chairs and glider stools, that had potential entrapment hazards. The program director removed the stools from the classroom and reported they were going to cover all entrapment areas on the glider chairs.

The Infant toddler room is located in the basement. The stairs to the outdoors are an obstruction to the evacuation crib, leaving non ambulatory children at risk during a evacuation.

Medication
At the time of the visit a child’s Benadryl was expired in the preschool classroom.
At the time of the visit the kitchen area in the preschool classroom was accessible to the children. At the time of the visit it was observed that the teachers area in the toddler room was cluttered, accessible to the children and there was a purse accessible to the children. The climbing structure in the Infant/Toddler room had duck tape on the rugs, that could be a tripping hazard. There were pipes in the toddler bathroom that were exposed. The rug covering the step leading to the bathroom in the toddler room was ripped and posed a tripping hazard.
